Realself was *instrumental* in my decision to get my breasts enhanced, so while I don't normally write reviews, I know how important reviews are in finding the right implant size/doctor. Thank you ladies!
Mostly writing this because there are very few reviews about over the muscle implants and I hope anyone considering overs vs unders can find it useful.
I'm a professional researcher in my day to day life so when it came to breast augmentation, my approach was no different. I researched for 2 years before booking my appt, watching testimonial videos on Youtube, pouring over hundreds of images and trying on endless rice sizers. Like many ladies on this site, I once had full beautiful breasts but my 32D/36C girls had deflated to a 32B or small A after losing 30lbs and getting into fitness. I had some sag and extra skin.
*My stats: 30 years old* ---> no kids
5'4, 115lbs. Petite athletic frame, 33, 25, 36.
Chest circumference: 13inches
Rapid recovery Over the Muscle Implants
*385 Sientra Round Gummy Bear Implants*
I chose Dr.Windle in Kirkland, WA (Seattle area) to perform my surgery. When I went in to see him, I had these firm set in stone notions of what I wanted, namely round smooth silicone implants, not high profile and under the muscle placement. I didn't want my breasts to look fake and I didn't want people to know I had a boob job. After examining me and seeing that I was athletic and had some sag to my breasts, he recommended high profile *over the muscle* placement with Sientra gummy bear textured implants. He showed me images of patients that had my similar build and demonstrated how natural over the muscle can look. I was initially resistant to this implant idea, but after doing some research I understand the benefits of 'over the muscle' vs 'under the muscle' placement.
Over the muscle benefits:
1. For girls that have some sag and extra skin, over the muscle helps correct sag without a lift. My sagging was mild/moderate so it worked for me but might not work for everyone.
2. Over the muscle implants move more like natural breasts. I have several friends with under the muscle implants and while they're lovely, they tend to stay in place at all times. They don't quite have the drape and bounce like natural breasts do. Overs move and act more like real breasts. Think Kate Upton- her breasts are fake but she has a softness and drape that looks very natural. Wiling to bet she got over the muscle.
3. As an athletic girl, I didn't want my gym routine impacted by my implants. I watched many Youtube vlogs by female lifters describing how their under the muscle BA limited their lifting routine.
4. Recovery: Over the muscle is *way faster* recovery wise. Today is day 3 post op and my pain and recovery has been a cakewalk compared to some of the girls I've known who've gotten a BA. 3 day recovery vs 3 weeks. On day 2 I hit the gym (lower body only of course).
Size: I was a 32b before but had extra loose skin from when I used to be a 36c/32D. I wanted breasts that would look proportional to my 36 inch hips/ petite frame but still look busty when I wanted to play them up. Overs look BIGGER than unders and going under the muscle you lose 25-50ccs from the compression. I initially started at 325cc, then jumped to 350, 385 and 400. 350 looked too small, and 400 made me look crazy, so 385cc it was. I lost many nights of sleep wondering what size to get. I thought 385cc was going to be crazy big but kept reading women say on RealSelf 'go bigger or you'll regret it'. My doctor told me anything in a 300-400 range would look great and not cause rippling, he was relaxing to talk to and his assistant Pam told me that the difference between a 350 was just 2 tablespoons, and it would only show up in sideboob, not projection of the implant. On day of surgery they looked insanely big, I felt I'd made a huge mistake. Today is day 3 and they've settled a bit and swelling has gone down, they look more natural and I love them. Moral of the story: go bigger.
Pain: While my procedure is called 'rapid recovery', it does not mean it is pain free. My doctor did not offer pain medication following my procedure. It was a Tylenol and Aleve only regimen. The first day my pain was intense, however this is normal and they tell you it will be that way in aftercare instructions. I had a friend find me the strongest weed edibles they could and I promptly fell asleep and felt no pain. Day 2 I was more or less fine, walking round and only feeling moderately achy at times. I even hit the gym! I have to hand it to Dr.Windle, it truly was a 'rapid recovery' surgery. My procedure was done on a Thursday and I could happily go back to work Monday morning without anyone knowing I had surgery.
The process: I checked in at 8am, nervous as all get out but within an hour I was being strapped down to the operating table and listening to music. I opted for twilight anesthesia, not general where they knock you out, and didn't have the grogginess or nausea typical of surgery when I woke up. I was pretty with it and within 15 minutes off the table I was wheeled to my car and on my way home at 10am.
Bloating: No one told me that it looks like you gain 10-15lbs after surgery from bloating alone. You may feel your boobs aren't as big as you thought they'd be, but its really that your gut looks 3x as big as before. It goes away in a few weeks. I weighed 114lbs on surgery day, its day 3 post op and I look like I just gave birth.
Stretch Marks: They appeared overnight and are visible around the top and bottom. Going from a 32b to (currently measuring) 34D or DD it's no surprise. Going to treat them with some nice oil and see if they fade.
Sensation: my breasts are still mostly numb, completely underneath, a bit on the sides and I have more sensation in one nipple than the other but they aren't completely dulled. It doesn't bother me so much given the trade off and my frame finally feels proportional, more womanly.
*What I learned from looking at 400 breast implant before and afters*: the women who got the best results generally did their homework. Find a girl who has your same measurements- weight, height, chest circumference and existing breast tissue. If you can find your [RS bleep] twin on RealSelf and put a fine point on general cc range and projection, you can take that to your doctor as a good starting place and let her/him use that info to give you the best result. I feel its far more effective to take photos of women who are your build with desired results to get a good outcome rather than asking to be a D or a C cup. Fake breasts look about a cup size smaller than natural breasts, no matter what the size. You're going to have to go a bit bigger than you want to get the results you desire.

Day 4 Post Op

The girls are dropping and becoming more natural, really happy with them so far! My nips line up, just a angle of photo makes it look a bit uneven. Had my post-op appt today, scar doesn't look too bad after they removed the steri-strips.

Its been 1 month since my BA and I feel great about the decision. I went from a 32b to what now appears a 34D, (haven't been officially sized but thats the size of my no wire bra). While it initially seemed very scary and nerve racking to get this done, I couldn't be happier. My confidence, even in non-revealing normal clothes, has increased tremendously and my body is now proportionate. Getting 385cc and having a small B before, I cant imagine how women go smaller than 400ccs. Augmented breasts look a cup size smaller in clothes than normal breast so I strongly encourage you to add 50-100cs to your desired implant size.No one can tell I got breast implants, no one. If Im wearing a top where I want to display them, (tight), you're going to need a push bra to announce their presence.
I will say, over the muscle looks a bit augmented on me but its because I weigh around 110lbs and there is not a lot of tissue to cover the whole upper pole, but it still looks great. My significant other who famously said he was 'not a boob man' over and over again, could not be more enamored with the girls. Will post 1 month update photo tomorrow.

You look great! I think they look very proportionate to your frame. I totally agree the implants look much smaller then what the sizers look like. Also if you go under the muscle, like I did , it reduces the size a pretty good bit. I got 485 but they seem much smaller to me.
