Hyaluronidase for Hideous Tear Trough Filler

WORTH IT
On July 10, 2017 I made a huge mistake of getting under eye filler with an aesthetic nurse at a wellness center. Because of a couple selfies in poor lighting I started to think that I had under eye hollows. She played into my insecurity agreeing with me and advising me to get Vollure of the Juvederm filler family as it came with 2 syringes and she thought I needed more than 1. Needless to say it turned out absolutely hideous: overfilled with huge bumps and ridges and unnatural looking dark blue “rings” under my eyes. It also created weird creases where I never had any creasing before the procedure. I developed malar bags as well. It made me look like a 100 yr old witch. When I showed it to her on day 5, she said it looked great and I needed to wait 2 weeks. Yeah, I looked “great”, alright...maybe if I were to go out for Halloween dressed as a zombie without having to do any make up! I told her I hated it and wanted it gone. She said she could take it out but the dissolver would burn and make my eyes red and super irritated, basically describing something out of the Exorcist. She then proceeded to tell me that she never even injected Vitrase around the eye area. At that point I was not about to let her touch my face and went home instead to do more research online over the weekend. There were only a handful of hyaluronidase reviews with positive outcomes and one of them was from Carmk here on RealSelf. Her doctor was Dr. Todd Hobgood from Scottsdale, AZ. Her review gave me hope and I contacted the office on Monday. Dr. Hobgood was booked a month out but they were able to get me in on July 20, 2017. I am so grateful they did because I could not imagine living with that mess any longer and now I know that it is best to not wait and to dissolve it as soon as possible. Filler stretches out the delicate under eye skin and it is hard for it to bounce back, I think that is the main contributing factor in all of the hyaluronidase horror stories on here.
Ashley, one of Dr. Hobgood assistants, let me speak with Annie Bruno, Certified Aesthetics Nurse Injector at their office. I told her about my experience and she was very reassuring and took her time to calm me down and let me know that they had been successful with correcting filler issues but it may take several rounds of hyaluronidase with cross linked fillers like Vollure, which is not the best product to use for the delicate under eye area. I really appreciated her input and she made me feel so much better.
I was a nervous wreck on the day of the appointment with Dr. Hobgood and almost passed out in the waiting area filling out treatment consent forms. I was called into one of the patient rooms, Dr. Hobgood walked in and all of my worries melted away. I immediately knew I made the right choice. He treated me with so much kindness and compassion, he didn’t even have to ask me what happened, he knew exactly what was wrong with my eyes and told me it was not going to get better on its own. The area was extremely overfilled. I told him that the original injector used a syringe and a half and he though that amount was excessive. I asked him if the hyaluronidase injections would burn or irritate the eyes and he said that none of his patients ever mentioned that before. Dr. Hobgood used Widease (not sure of the spelling), I did not feel a thing, no burning, no redness, nothing... I could not believe it. It was absolutely painless. I could immediately see the difference and those horrid blue rings were disappearing.
I had a total of 5 rounds of Widease with Dr. Hobgood over the course of 11 months. The new generation of cross linked fillers is made to last longer making it more difficult to get rid of. Dr. Hobgood did an amazing job. He saved my eyes and my sanity! I definitely hold him in high regard and tell everyone how amazing of a doctor and a human being he really is. All the beautiful ladies at his office are really sweet and thoughtful as well. I am so grateful to all of them!

Hi there, you’re so fortunate your story had a happy ending. I know you said you had 5 rounds done to dissolve it. Can you please tell me how many units total he used over those 5 rounds?
December 22, 2018
Hi DAS13, I just replied. But I think it posted as an independent comment under the original post. Let me know if you see it and it if makes sense.

Hmm... Good question... I don’t know about the number of units but I do know that a normal dose was 0.7 Cc. I believe that’s how much Dr. Hobgood used on me for the most part. When I flew in for 3 days on my second trip to Phoenix, I got 2 hyaluronidase rounds a day apart: 0.7 on the first day and 1.2 cc on the second day. At that point I developed malar bags from the filler and Dr. Hobgood thought that “blasting” those areas would take care of that issue. It definitely worked. I am not sure if hyaluronidase is diluted with anything or not but you could call Dr. Hobgood’s office. Everyone there is super nice, I’m sure they would be able to answer that question. I hope this helps:).

Thank you for the reply! Sounds like you got 1.9 total which is about 360 units total. I had
.5 used total which is about 100 units. After the dissolving I lost fat and volume under my eyes and my upper cheeks. It has been 2 months and none of the fat on my upper cheeks has come back. Do you feel like you experienced any volume loss like that or no? Considering you had many more units than myself you are so lucky. Most people that had positive experiences has 50 units or less total.

As far as dilution mine was .5 hyaloranidase and .5 saltine dilution. If yours was half saline of course then it would have been about 200 units hya which is still double what I had for units of hya. Please let me know if you had any volume or fat loss. And if so if any ever came back or no?

Hi DAS13, yeah, it sounds about right as far as diluting goes. I think I actually had more units than that at 0.7 cc X 4 times and 1.2 Cc once. I can with 100% certainty say that I did not experience any volume or fat loss. I asked my doctor about that and he said that natural hyaluronic acid restores in your body naturally, I think he said it was a 24 hour or a 48 hour cycle. I did have the appearance of some skin laxity and puffiness following the Widease injections but it wold resolve within 3 months. I used L-lysine ointment for under eyes (not sure if it’s the right thing to do but it seemed to help with skin elasticity) for a few weeks and did eye exercises (facial yoga stuff on YouTube) to get he excess moisture gathering in that area out. I felt that persistent puffiness was my main issue. I felt there were remnants of filler left that constantly attracted moisture to he area, especially in the morning or after having any alcohol the night before. Once I discovered those pattens, cut out alcohol and did eye workouts, it was easily fixed. My fifth round seems to have taken care of any remaining filler. I also think the volume loss may have something to do with the brand name ( although unlikely as it is probably all the same active ingredient). My friend said she had Vitrase done to dissolve a filler bump by her lip (done by the same nurse lady that did my filler injections) and she said it really burned. I know my Dr. uses Widease. Widease did not cause any pain or discomfort whatsoever. The Dr. I later went for a consultation with here locally used Hylenex. I decided to not risk it and ended up flying out to Phoenix one last time since I already knew how my body reacted to Widease.

That’s a good point. Maybe it’s possible that different brand names have different affects. Bit tase was used on me. It didn’t burn, however, I lost so much fat in my cheeks after it and it dissolved 90% or more of the filler in just that one treatment I had consisting of 100 units. I just don’t look like myself anymore. And I can’t stand to look in the mirror. I use to love my face now it is deflated, can’t believe this. Also although hyaloranic acid regenerates. HA binds collagen... I’ve been reading studies and hyaloranidase is used to remove fat in some new products being tested. You’re so fortunate not to have been affected.

I had 3 syringes of filler under my eyes. And the 100 units (.5) vitrase dissolved nearly all of that filler.
December 24, 2018
Omg! It sounds horrible. Do you have a picture? I’m really sorry you are going through that. On the other hand, 3 syringes sounds like an awful lot of filler! I had a syringe and half of Vollure and my doctor said that was way too much and about a third of that amount should have been used in the first place. Now I realize that I never even needed the filler in the first place. It was one very expensive mistake...

Same as you I never needed filler under my eye. I went in just for Botox and dr suggested it. He told me there were no side effects or anything that could go wrong lol , it was stretching out my skin. And I didn’t want my skin to be ruined in a year later from it being so stretched. I went to a different dr to have it dissolved and asked him if hyolaranidase could damage my skin. He said it would only be dry a few days then be fine. So I told him to just use a minimal amount 15 units each eye. Within minutes after over 90% of filller was gone. By the next day my cheeks were missing and under my eye fat. I had a really really full face before and a lot of cheeks. He told me it’s impossible and I’m seeing things because HA would only effect skin texture. The thing is HA binds collagen and I know my face - the fat is gone. Also, I asked him to write down how much he used and he had trouble figuring it out. Finally found out it was .5 vitrase mixed with .5 saline which is way more than I asked for - 100 units. 2 months later and no improvement. I never wanted to do filler again but I hate looking at my face. So I had half a syringe of volbella divided under each eye. Which is barely any but I wanted a little evenness above my cheeks. Now that I’m reading about volbella I’m seeing it causes a lot of nodule and delayed reaction issues so I’m really worried. I just had that 3 weeks ago and I caught a cold/sinus infection. I read if you get sick within a month or during filler injections you have a high risk for it getting infected down the line. I just can’t believe all this and am so scared what’s next.
Your so lucky none of your facial fat dissolved. This is all a nightmare.

Oggly you mention puffiness after wydease the 1st time you go it injected and it or every single time and it would last 3 months? I ask Because I have had 4 rounds of vitrase ( undiluted) and the puffiness is STILL THERE I am going crazy :-( and I am wondering if at least you saw less and less puffiness after your treatments. I got refyne restylane and I am just learning it had 4 levels of CROSSLINKING. Ugh I hate it!

Hi Sincere6779, I feel like every single time after injecting widease it wood get puffy and stay puffy for a while until about the 3 month mark. The skin texture seemed weird too, dough-like. I hated the look of my eyes after the dissolver injections because it seemed to me it looked worse that it was before. Then at about the 3 months everything wood return back to normal and I could see where the filler still was. Every time is wild be less and less. My final I injection took care of it for me and just like that I stopped thinking about it. I hope it helps. I had a total of 5 rounds. The filler that I got was Vollure. Super strong, Cross linked, just like yours. A giant pain to get rid of. It was a long journey for me. It will all turn out okay for you! Maybe try making an appointment with Dr. Hobgood. He really knows what he is doing.

Thank you for taking the time to share your experience. It’s hard to overstate how distressful it is to deal with this! Your experience is encouraging but there are so many negative experiences posted here that I’m still afraid of making things worse. You mentioned doing eye exercises, after your last treatment did you get completely back to normal and no longer having to worry about diet etc? Did you experience any bruising with the Hyaluronidase?

Hi jeankim, as far as I can tell my eyes are back to normal. I feel completely normal now and not self conscious, I don’t even think about my eyes so it tells me that they must be back to how they used to be before the filler. During the 1.5 years after the filler my eyes were ALL I could think about on a daily basis. I no longer worry about diet and don’t have to do eye workouts. Skin texture is back to normal as well. I typically did not bruise from Hyaluronidase but there was one time where the needle hit a capillary near the outside of the lower eyelid and a tiny area bruised. I just wore eye liner and no one noticed :).
I know it is super hard but it can be fixed. My doctor used Widease, I don’t know if that makes any difference in the outcome. Let me know if you have any more questions. I really hope you can find the right person to fix it for you.

I am actually going to see Dr. Hobgood but can’t get in until the end of next month. I really appreciate you sharing your experience. Seems like this is not an uncommon problem and yet there are very few doctors who seem to know what to do. I hope to be as fortunate as you.

I am so glad to hear that you are going to see Dr. Hobgood! He is truly the best. Are you a local or would you have to travel? In hindsight spending a lot of money flying out to Scottsdale to see Dr. Hobgood was the best decision I made. He is very, very kind. Even though I was a pretty low revenue-generating client in the scope of his practice, he spent a lot of time with me explaining things that other doctors won’t tell you about tear trough filler, the nature of the filler, how dissolving works, etc. Another thing that stood out to me is that Dr. Hobgood never tried to take advantage out of the horrible situation to bring up any additional procedures, like adding more filler after dissolving, like I heard some other people on here say. He is just very honest and does not have the need to ”up-sell”. That’s why he has so many loyal patients. Had I known what I know now I would have never done the whole filler thing in the first place. I think it’s the most difficult procedure to get right. There is just no exact science and it seems that there are more people that end up with undesirable outcomes than the ones that rave out it. As far as the wait times go, I would call and ask if there are any cancellations, so maybe they could fit you in sooner :). Please let me know how it goes after you get it done, ok? Keeping my fingers crossed that everything turns out great for you!!!!
I think the doctor’s skill and experience with dissolving poorly placed filler plays a huge part. I think hyaluronidase needs to be injected in just the right spot and at the right depth. That’s why I picked Dr. Hobgood. There were only two Hyaluronidase reviews for him but they both sounded very happy with the result. Dr. Hobgood is in Scottsdale, AZ so I flew out to Phoenix 4 times. The dissolving cost me a lot more than getting the stupid filler.
From the nightmarish hyaluronidase reviews I read on RealSelf, it sounds like the people that had sagging skin and other issues are typically older and the skin may be lacking elasticity to bounce back. The other issue I think could be that they have had filler under their eyes for too long where the skin got too stretched out and once the volume was removed it started to sag and appear wrinkly. Another issues that comes up often is dents or uneven appearance and that may be attributed to the doctor’s skill, bad use the filler was not dissolved all the way and when what’s left of it swells up and in contrast to the rest of the eye appears dented.
When I saw my horrible filler result the day after I got it, I started moisturizing my under eye area like crazy so it would not stretch out. I walked around with that monstrous filler for 10 days, so glad I had the common sense to book my appointment right away (which I had to wait for, otherwise I would have dissolved on day 3) and to go to a right person and not wait at least 2 weeks/month like the nurse who did it to me suggested.
Hyaluronidase adds liquid to the area and immediately after injecting it looks pretty good but the next day it makes skin around the eyes looking dough-like, a bit bumpy and kinda saggy. I would say It takes about 3 weeks to resolve and 3 months to get to totally normal, just from my personal experience. The biggest problem throughout the dissolving process is persistent swelling. But I figured out how to get it under control by doing facial yoga eye workouts :). Also a good moisturizer is a must. Now my skin and my eyes are back to normal. No one would have ever guessed I went through something like this.
I think you will be just fine! It might take a few rounds to get it looking the way you want it to look but it can be done! The most important thing is to pick the right doctor who has a lot of success with dissolving filler. I really hope this helps! I so remember myself being in the same situation, crying, not being able to sleep at night while agonizing over making the right decision.
May I ask you what type of filler you got?
