Just like many other stories I've read here in RealSelf, I made a foolish mistake in 2009. I was introduced by a friend to a nurse who performed injections to enhance buttocks. She claimed it was PMMA which was reabsorbed by the body as times passed and even had an entire gallery of her before and afters. Now I wonder how many of her patients are going through same regret as myself. Also I had multiple friends who had it done and also wanted to have a curvier body. I went ahead to have the injections in one session. I am petite, 5'2 110 lbs. After 2 years passed I had no complications whatsoever. But I eventually formed a discoloration between both of my buttcheeks and hardness. I asked one of my friends and she advised me to just massage. I lived like that for the past 3 years. I started feeling more discomfort if sitting for longer periods of time. So I decided to search online for a doctor with knowledge on the field. This website helped me find Dr. Raffi Karamanoukian. I booked an appointment and when I met him I knew there was hope. He was very compassionate and understanding when I explained my story. He went on to explain the reason for all my symptoms. He also showed me other cases of silicone mistakes. I met with him two times before I booked my surgery to partially remove the granulomas I had developed. Now that I'm almost a month after surgery I can say there is improvement. The doctor explained I would have swelling for a little while. I definitely feel better. I can say I was very lucky to be alive. I advise any other woman to do their research and never let anybody else other than a board certified surgeon do anything to their body.

No I feel normal. Still a little sore in the middle. The incisions are hidden, (they are at bottom & on each butt cheek). Have you consulted any doctors?

Hi thank you for responding. .. I did consult one doctor so far and she said she felt 3 lumps and that if he remove it then it will leave dents and holes, that's why I was asking if u have any dents?

On my post op consult Dr. Showed me the lab results. Which stated it was liquid silicone. I Even though not a big amount was removed, I do feel better. The discoloration between both butt cheeks was also hard at touch so I think that's were my body had accumulated most of it. About 3 inches wide and 3 inches tall. I will continue to monitor myself with him and I was told if any future complications arise, then I would need an MRI. Plenty of doctors even refuse to help you so I am very glad I found this doctor.

That is the same thing that is happening to me.. by the Crack is where there is a big lump and it's peel and Lil hard... so when he took out lumps there was no holes or dents? The doctor I went to see said it there Will be.... thank u for taking time to answer my questions

There are many tests that can be done to assess the degree of inflammation and tissue reaction. My office works alongside a top immunologists and allergist in Los Angeles to assess options for immune modulation with severe tissue reaction.

The cost of silicone removal depends largely on the severity of the inflammatory reaction, volume of injection, and the risk profile of removing the tissue safely. Not all patients are candidates for removal of the silicone.

The volume of silicone removed is often hard to assess because there is associated inflammatory reaction and fibrous tissue. Total volume removed does not correspond to volume of liquid silicone removed in many cases.

Silicone can cause problems in the soft tissue, mainly due to the inflammation that arises from the contaminants within the silicone polymer. Some of my patients have physical signs and others only have symptoms associated with the silicone rejection.

MRI and CT scans can be of immense use when the silicone is in the muscle and fascia. Lab tests can assess your suitability for surgical intervention. Good luck.

The decision to undergo cosmetic reconstruction after silicone injection removal is a complicated one and oftentimes, it is necessary to approach fat transfer and grafting more conservatively in order to minimize further problems.

I have been doing lots of research trying to find a dr. from Los Angeles that can remove silicone from my buttocks, thanks so much for giving me Dr. Raffi's info, I'll keep you posted, I have high hopes, I think my case is way worse than yours was my butt looks shapeless and one butt cheek is bigger than the other, oh boy, what a disaster, I feel like dying everyday, thanks again!

There are many options available for those patients who are suffering from silicone reaction. Unfortunately, some patients wait too long and have advanced tissue reaction that does not allow for successful removal without scarring. Early treatment is key.

I see many patients with silicone complications and have used a classification system to categorize complications with silicone injections. Skin reaction is often an early warning sign of inflammation and local tissue reaction.

Can the lumps be broken and then suctioned out? Would this make decrease the tissue reaction? I was told my a plastic surgeon I can get this done...

Thanks for info. What would be the solution? I'm on prednisone but I can't take this forever because of the side effects. Would anti-biotics or an immune suppresent be the next step?

Hi vee, I actually just spoke with the doctor who said he's willing to remove it. He told me that I have a lot in my butt and that he's going to take out as much as he can, he also told me I will be left with nothing after and that it depends what he encounters when he opens me up if I can get a fat transfer. Some doctors will remove the lumps and scrape and some doctors will remove all the tissue... I believe if u still have silicone left in your butt no doctor would do a fat transfer. But I'm going to take a trip down to Dr gallerani in FL to see what he has to say. It's a really tough situation bc they can say probably and then it can be a different outcome after surgery bc they have no idea what they will encounter when they open u up... how is your friend doing? And it also depends if the silicone is in your tissue or muscle or both

It is oftentimes very difficult to remove silicone from soft tissue. Liquid silicone that we remove in our office is sent for pathologic examination, and in most cases, there is evidence of granuloma, fibrous reaction, inflammatory reaction, and foreign tissue reaction in addition to the foreign material. In my practice, I have a very large series of patients who have undergone successful removal of silicone from the body.

Thank you for your comment. The removal of silicone from the soft tissue is complicated and your candidacy for surgery cannot be made by diagnostic imaging alone. Although an ultrasound is useful, it does not replace a comprehensive examination. I have developed and continue to advocate the Santa Monica Grading Scale for Silicone complications as a useful tool for the treatment of silicone problems.

Thank you for your comment. Eczematous patches on the skin is usually a precursor to a more advanced silicone reaction within the soft tissue. Inflammatory skin reactions are difficult to treat unless the underlying problem is addressed, namely the foreign body.

I had silicone removal on Dec 11 2014 with Dr.gallerani in Florida. I could honestly tell you that it was the best decision I made. He removed scarred tissue and more than 90% of the silicone from My buttocks. Even though I don't have a big butt anymore, I can now live a healthy and productive life.;)there is HOPE u just need to find the right doctor... God bless all of u...we did all make a mistake but now we need to just learn from it.

@ketit1983 hello & God bless, how long was your healing process, for the scarred tissue, did he cut you open to remove, and did it leave any deformities, how much did it cost, I've heard he is very expensive? Are you planning/considering or can you get fat transfer in the future? I would trully appreciate your response, and btw, thanks for sharing. God bless!

The issue of fat grafting is a common question we face when dealing with silicone related complications in the buttocks. Although it would seem that the replacement of inflamed silicone with viable fat may be a good option, it is not necessarily a longterm solution for patients with Grade 3 or Grade 4 silicone reaction. These patients will likely have longterm complications related to fibrosis and inflammation, and the injection of more fat/viable tissue into areas of inflammation may be a poor choice.

My pleasure. The most important tasks in treating silicone complications are to first treat the medical issues while preserving the cosmesis of the soft tissue. Elective cosmetic surgery is an important, but second priority.
