- Quick update here, sorry for the delay I've been super busy!
- 34 days post the operation today
- overall I'm very pleased with the outcome so far, I no longer have so much asymmetry in my nose, it flows much better now, the shape is also much better
- it's getting better everyday I look at it too, I'm already very pleased with the outcome of the operation at just 4 weeks and there is more refinement to come
- breathing has definitely improved. The collapsing has gone so I can breath much better now
- I am getting congestion from time to time but for the first time in about ten years I have been able to sleep without nasal inserts.
- I still whistle on inhale on the right nostril from time to time, I'm not sure why
- I had a pretty decent recovery, first week was the most difficult
- I'll post some pics when I have time!

Revision Rhinoplasty February 29th 2024

Hi all,
I will be documenting a revision rhinoplasty that will be performed on February 29, 2024, with Dr. Robotti. This will be my fifth rhinoplasty, and I hope that it will be my last! Overall, I’m excited to be in a place where I can hopefully soon put this all behind me and get on with my life.
### My Aims ###
- Straighten the nose (deviated Septum and flow of dorsal aesthetic lines, my nose is crooked and leans to the right of my face)
- Improve breathing (Stop collapsing on both nostrils, eliminate whistling on inhale)
- Alar retraction right nostril (nostrils are different shapes from the 1st rhinoplasty that changed them, can see into the right nostril on frontal view)
- Better symmetry in my nose
- Tip debulking (the tip is now wider than pre any rhinoplasty I had, it’s noticeable when smiling the most)
### Dr. Robotti Aims ###
- Fix the deviated septum
- Provide structure to each nostril to improve breathing and nostril collapsing
- Reduce turbinates size (he identified these as one of the reasons my breathing is bad)
- Return a ‘pyramid shape’ to the nose, so that it flows better (dorsal aesthetic lines and bony vault work). This will slightly improve above the tip as well where bones are crooked.
- He may fix the alar retraction, he wasn’t sure in the consultation about this, but he has noted it as a ‘maybe fix’
- He may need to return a proper ‘L Strut’ to my side profile
### Initial Consultation ###
- A few weeks ago, I flew to Italy to meet Dr. Robotti. He is a great guy and came across very knowledgeable and confident that he could improve the issues I face. I could tell from speaking with him that he is an expert in fixing noses and I’m confident that I will be in good hands in February.
- I really liked the consultation process, which included a scan of my nose, which showed that my septum is deviated (happened originally with trauma from a fall) and surfaced other problems that I’m facing. No other surgeon has taken this approach to understand the underlying nose structure before a rhinoplasty with me. I felt that this is essential information to gather for a successful surgery, especially where a nose is crooked to determine the correct course of action for a surgery
- He spent a huge amount of time explaining the root causes of my problems, he was also extremely honest about what he could do and couldn't do (something that I've learned is really important in plastic surgery).
- Overall, I left the consultation feeling that I was in good hands with Dr. Robotti and that he was the surgeon to finally fix my issue after a long journey through this over the years.
### History ###
- My original nose was straight but I had a drooping tip from the side view, I never liked the drooping tip.
- I had an accident when I was about 19 years old that resulted in a crooked nose
- The first rhinoplasty had 3 main goals a) lift the tip, b) straighten the nose and c) change the nostrils. After this surgery I had a very unsymmetrical nose and no attempt was made to straighten it, the alar retraction issue started here as well.
- The second rhinoplasty, I asked to a) Straighten the nose, b) Fix the unsymmetrical things (nostrils, lumps, bumps etc). After this surgery, the physical look of this nose wasn't too bad but unfortunately after this surgery I was left with both nostrils collapsing, resulting in me snoring loudly. My deviated septum also was not fixed.
- The third rhinoplasty was to improve my breathing, the surgeon added spreader grafts. I asked a) Fix the breathing and b) Straighten the nose. Unfortunately after this I ended up with pollybeak deformity and a larger nose.
- The fourth surgeon, I asked to a) fix the pollybeak deformity b) straighten the nose c) improve the breathing and d) remove the excess bulk that has been added to the nose. They did fix the pollybeak deformity, they did not straighten the nose, they made the nose wider again to improve breathing
- Since the start of this my nose is now wider than pre-surgery, both in the bridge and tip. It seems like my previous surgeons were edging on the side of “a bigger nose breathes better”, instead of fixing the underlying structural issues with the nose cartilage.
