At the surgeons request in her reply to my original review I am posting before and after comparisons of my nose. As I said in my review there are some minimal improvements but there are things that are worse now which has caused problems that did not exist at all before my revision.
I would also like to add that Dr. Harirchian did reach out to me through email after my review was posted apologizing and wanting me to come into her office to discuss what can be done to improve my results and make me happier, it is a little disheartening that it took the review for her to feel like she should reach out to me and not just doing so out of her own concern from how things were left at the last post op.
Long review but please read before choosing this surgeon.
After having my nose completely botched during my primary rhinoplasty by another surgeon I was left with asymmetry, difficulty breathing, a collapsed/receding nostril, twisted tip, and hump on my 3/4 profile. When I started researching for a revision I consulted with several top surgeons in Texas before being recommended Dr. Harirchian. At my previous consultations with other surgeons I was told the issues could be fixed but that also trying to do more of the cosmetic changes I had wanted with my first surgery wasn’t realistic. I should have listened. Upon meeting with Dr. Harirchian she was EXTREMELY positive that not only could she fix the issues I was having after my primary with no problems but that she could also give me the slope, deprojection, and alar base reduction I had wanted before. She also mentioned that I had enlarged turbinates and that was what was causing my breathing issues and those needed to be fixed, I asked her to include that in my surgery.
Immediately after my splint removal I was happy, I was very swollen but my profile looked amazing. At this time she told me she didn’t feel comfortable with doing the alar base reduction during surgery but she wanted me to come back at a later time to do it in office at no additional cost. I informed her of a lump on the inside of my left nostril and a small bump inside my right. She told me the left was swelling and she believed the right was a stitch that could also be fixed at a later time. A few weeks later I noticed the dent in my nostril coming back and immediately emailed her with photos. I was told she did “extensive” tip work and not to worry that the sides would even out and be more symmetrical. For the next few months it was difficult to get in to see her, the couple times I did see her she was very sweet and assured me again things would even out and that the issues inside my nose were still just swelling. I started having difficulty breathing in the left side I believe due to the lump inside of my nostril, as you can see from the photos it is blocking a very significant amount of my nostril. My columella was starting to look twisted, the dent was getting worse and now the hump at my 3/4 had started to show itself again. At the next follow up she was saying the lumps inside my nose were scar tissue but not to worry because she could just “cut them out” in office and not even have to be put under. She also made the remark that she was glad she hadn’t done the alar base reduction yet because my breathing would be worse because of the left nostril being so swollen and blocked but we still could do it in office when everything settled in my nose. She gave me a steroid shot in the hump on the side of my bridge and told me to come back in a few months. All the issues I was having only got worse in this time. On the day of my last appt which put me around 7 months post, my husband, who was with me at every single appointment before was “coincidentally” not allowed in the room with me. Dr. Harirchian attitude toward me completely changed. She told me the lump in my 3/4 I was seeing wasn’t there at all and showed me photos the office had taken but from the opposite angle. After I informed her it was actually from the other side that the hump was visible she finally acknowledged the hump and said another steroid shot was needed. I again told her I thought the lump inside my left nostril was causing my breathing issues and she said that wasn’t possible and that it was actually graphs she had put inside my nose and not swelling or scar tissue like she had previously said multiple times, she informed me I was stuck with them because my nose could collapse if she removed it and no one would ever notice them anyways. This contradicts everything she has said before. If she knew they were graphs all along why did it take so long to tell me? Why did she continue to say if the issue didn’t resolve she could just remove the lump in the office without anesthesia?? And why did she keep telling me the alar base reduction was still possible at a later time when swelling in that nostril subsided? So was she lying then or is she lying now? She also told me again my difficulty breathing was being caused by enlarged turbinates. Why were my turbinates not fixed during surgery like we discussed in consultation considering my breathing was one of the reasons for my revision?? When I told her the dent was getting worse and I felt very insecure about it she told me that “she knows I like to take pictures” (she is aware my husband and I have large social media followings.) but that I should be grateful because of how crooked my nose was before she fixed it. I told her I felt like she had lied to me about what she was capable of and she then left the room returning with a cell phone asking if I had a weak stomach. She proceeded to show me pictures of my nose on the operating table cut open so that I could see how “awful” it was before and all the work she had to put into it including the graphs she placed along the tip from cartilage in my ear. In tears and upset I told her again that my results were not at all what she had promised me and I felt like she wasn’t trying to help me in any way. It honestly felt like bullying and scare tactics. I also told her it was terrible that she expected me to just live with my nostril being blocked. She said that if I’m having trouble with breathing I should get used to using nasal spray and if the dent/collapse bothered me that bad I should just get filler. After she gave me my 2nd steroid shot (which as you can see has still not fixed the issue with my 3/4 profile) she handed me a tissue, told me to keep in touch, and left me crying in the room by myself. She did not offer a follow up appt or any other resolution.
I do not think she should agree to do extensive revisions or make promises that are unrealistic. She is extremely nice to those who stroke her ego but unfortunately when you question her or convey your unhappiness she becomes combative and unprofessional. (I have spoke to other patients that have agreed with this as well) I would also like to add that the same day as our last follow up appt she decided to unfollow me from instagram as if she was throwing a tantrum. She seems to care more about her social media presence and how many new patients she can push in and out than she does the patients she already has. Getting in touch with her or anyone from that office once your surgery has been done can at times feel impossible. And if they cancel an appt with you no one will reschedule so you will be waiting weeks or months to get in.
I am now left with the problems I had before paying her to do my revision. Major asymmetry, Dent/collapse on my right nostril, twisted tip, and hump on side of bridge which makes my nose look curved from the front and with hump and droopy from 3/4 view. Now I have new problems as well. Columella twists to one side, small bump inside right nostril, and large lump inside left nostril that is also visible from the outside and that I believe is blocking airway. I plan on seeing an ent soon because my left side bridge, which has the lump inside the nostril has started sinking in when I inhale. (I do have minimal improvement because my profile was deprojected and has a nice shape, specifically on the right side but we all know she is capable of that from the pictures she shares) I already had regrets and stressed over my nose due to my primary surgery and now I will have to go through all of this a third time because of the promises she made to me that ultimately led to a failed revision.
If you do your research and speak to MANY other patients of hers you will find this is absolutely not an isolated incident.
