Revision Rhinoplasty Procedure Diary January 2022

Intro
I'm starting this review to keep track of progress of my revision rhinoplasty that I had on 2022-01-06. I'm currently 7 days post operation and have uploaded some pictures of pre-op vs post-op. I'll continue to keep this updated with more pictures as frequently as I can as the swelling reduces.
Rhinoplasty History
I had 3 Rhinoplasty procedures before, the main goal of all of those Rhinoplasty procedures was always the same, to straighten the nose and fix the dorsal aesthetic lines. I was unfortunately not able to achieve this in any of them. I had some other refinements I wanted but the main thing I would have been happy to achieve is a straight nose with nice dorsal aesthetic lines.
Overall Social Experience so far
Overall the social experience so far has been quite good. Dr. Reish and his team are extremely nice and well organised. I received messages from Dr. Reish on two occasions after my operation to see how I was feeling, that was nice to know I can reach him at any time and it showed he cared.
List of pre-op issues
Below is a list of the following things I discussed in 2 consultations and before the surgery with Dr. Reish that I would like to see improvements in:
- Crooked nose
- Dorsal aesthetic lines are off and are unpleasing, especially in daylight of strong lighting (camera flashes are bad)
- Bridge was too wide
- Tip was too wide/bulbous and drooped somewhat when smiling, wanted more refinement
- Hanging columella
- A bump near the tip
- Breathing issues, nasal collapse completely on the left valve and somewhat collapsing on the right nasal valve
- General asymmetry on the frontal view
Cast Removal
I had my cast removed on 2022-01-13 and as expected the nose is extremely swollen.
Overall Experience with different views
Frontal View: Extremely swollen, really hard to tell anything about the tip changes or bridge changes at this point, it's currently larger than ever so I believe we will have to wait at least 6 weeks to start seeing some of the swelling come down and get a bit more refinement. I do notice that the nose still leans to the right and the dorsal aesthetic lines are not as pleasing as I’d hoped, they look to be the same as pre-op currently, I've discussed with Dr. Reish and he suggested to wait until the swelling reduces to see improvements in this area.
Profile View: From the profile view the slope of the dorsal looks much nicer, the hanging columella was also addressed in the surgery. There is a slight bump at the top of the bridge which can be seen in the pics, I discussed this with Dr. Reish and he noticed it as well and we are hoping it's swelling at this point.
Breathing: When my nose was cleaned out at the cast removal, I had the best breathing I’ve had since I injured my nose causing it be deviated/crooked in my teens. A few hours later it seems to be blocked again and I’ve been told not to blow it for 2 months. I’ll pick up some nasal spray and this should help with clearing it out and show us the breathing improvements. I had spreader grafts and batten grafts inserted due to the collapsing, so I’m hoping that this issue is now fixed and will get better in the long term when swelling reduces.
Overall Summary
Overall I'm really hoping that the bridge becomes narrower, tip gets smaller and more refined and the nose gets straighter with pleasing dorsal aesthetic lines over the next few months. I'm not sure if this was worth it yet but Dr. Reish is extremely re-assuring and is available to discuss all issues in our follow ups. I'll keep everyone here posted.

Adding an image that shows the crookedness, width and asymmetry really well for now. The tip is really large and so is bridge.. I'm not fully convinced that swelling will sort out the issues I'm seeing here, but let's see.

I've been thinking of that but what does it truly achieve? I'd really like my money back so I can get a revision of what I asked for

Do you think Reish would be amenable to that? He should refund you if you truly are “botched” once swelling goes down.

Have you reached out to the doctor with your concerns? I am certain that he can see that this was not what you wanted. If he is a respectable doctor then he should try to give you a revision or refund your money. And if he’s not a respectable doctor and refuse then I would look into getting a lawyer.

In my experience these surgeons never admit you're botched. They will say "it's an improvement", even when it's not. Surgeons in these types of fields are not held to be accountable for messing up something, or not doing what they were supposed to, it's usually covered by you agreeing that there will be could be some asymmetry after the surgery.
I don't believe I have any improvement here, even my left nostril is starting to collapse again now that swelling is coming down, so there isn't even the breathing improvements I thought I had.
I don't think I'll be offered a refund, I think I'll be offered a revision but if he couldn't do it the first time after we discussed it 3 times, I don't think he is able to do it in the next surgery either.

I agree. I would not go back to this doctor for revision. Please give this doctor a bad review as it will help others know to avoid him and for him to take responsibility for future botched surgeries. Good luck and I hope you will find an excellent doctor that will help you with the revision.

I had a revision by dr pearlman in nyc in 2015 and he does nice mens noses. I had a terrible nose job that went crooked and collapsed and he fixed it the best he could…it’s not the button nose I wanted but it looks way better than yours. Go see him.

Bump on bridge

Adding one picture of the bump on the bridge, I think it's a bit more pronounced today as perhaps the swelling has subsided a bit more. The bump wasn't there pre surgery for anyone wondering. We can monitor how it changes over the next few weeks.

As for a revision, I'm already thinking about it and would love people to share surgeons they know of that can actually fix deviated noses
