I am 46 years old, and in good health and in good shape. I had really pronounced lower lid bags with malar mounds as well (a plastic surgery challenge). I also had a bit of a double chin due more to genetics and a shortened jawline than to weight.
I spent the last year researching surgeons, comparing photos, asking questions, and had met with 2 other plastic surgeons before meeting with Dr. Mao. I was becoming very frustrated with my search as I noted that more often than not, the "before" photos of real cases were often in a different light and taken at a different angle than the "afters". The slightest change in light and head tilt can have a dramatic effect on the perceived outcome and make the results look more dramatic than they truly were in reality.
This is what really sold me with Dr. Mao. Not only is he kind, compassionate, knowledgeable, and takes his time to explain everything, he is also immensely skilled at his art. Furthermore, his before and after photos are taken in the same lighting and at the same angle so there is really no room to question the validity of the results he is presenting. And his results are phenomenal.
I am so glad held off on surgery until I met with him! After a comprehensive consultation and a discussion of what I was hoping to achieve, we decided on a lower blepharoplasty (lid lift) and chin liposuction. At no point did I feel rushed or pressured to do more extensive work. Dr. Mao clearly explains what can reasonably and realistically be expected with each option.
I had both procedures on the same day with local anesthesia and oral sedation (tranquilizer and pain killers). The worst part was the sting of the injection of the local anesthetic. After that I had ZERO discomfort. My recovery was fast and relatively painless. My eyes felt a little tight as did under my chin but the compression garment for the liposuction helped and ice on my eyes soothed any minor discomfort. I took one week off from work and my bruising was diminished enough that my glasses and a scarf covered any residual when I returned the following week. I followed his instructions clearly and precisely and I had no surprises or unexpected events.
I am now about 1 month out from surgery and I am very pleased with the results so far which will only continue to improve over the next 2 months. My bags and double chin are gone! Dr. Mao has answered every question I have had along the way quickly and completely. I would recommend him without hesitation. He only specializes in facial surgery and he truly is much an artists as he is a surgeon. I am considering more work in the future and will 100% be going back to Dr. Mao for anything further. His office staff is kind, considerate, and really go out of their way to arrange appointments and accommodate their patients.

Thanks! I am soo happy with the results. If you see my other posts you will understand what I went through to fix these. Fillers just didn't do the trick because my flaws were so extensive. while I wish I did this years ago, I am thankful I didn't until now because I woud not have had Dr. Mao.

Are Mylar Mounds also called Festoons? I think I have a small one under one eye. I
have heard these are a challenge. How did your Doctor resolve it? Was the lower bleph enough or did he do another procedure too?

Thanks! I had the lower Bleph however it was pretty extensive as he also tightened and reinforced one of the tendons supporting the lower lid (canthopexy) There is always the chance that fluid could collect there again but not to the degree it was preoperatively. I am not sure if festoons ate the same as malar mounds.

Did your surgeon do a transcutaneous incision on the exterior of your lower lid or a transconjuntival incision hidden inside the lower lid?

Transcutaneous "pinch" approach. My issue was more loose skin and fluid collection than herniated orbital fat.

It depends on the underlying issue. If it's fat herniation causing the issue then I believe they approach is usually transconjunctival My issue was loose skin and fluid with only a little bit of fat which did not need repositioning so that's why he did the approach he did. Don't be afraid to ask your surgeon for a detailed explanation.

Sorry. I just saw this now. I have not logged on in a while. The answer is yes. The crepey skin under my eyes was much improved after the surgery. Any residual lines or crows feet can be addressed with botox.
