Ok, this a lot to read, but you need to hear it if you've lost hope in finding a rhinoplasty revision doctor. I started my nose trial and error's by getting a bump removed by Dr. Fodero, in Livingston, NJ. For background, I wasn't blessed in the nose department. People said it wasn't that bad, but it's not how I felt and all the bullying in high school didn't help anything. Dr Fodero's work was a little bit messy, he didn't totally botch me, however it wasn't great work either. Improvement yes, but Dr Fodero kind of made my nose look more crooked and there was still a slight bump . I guess he shoots for subtleties and that may work for some's taste. I did a lot of research, talked to a girl who went to him, saw mostly good results, so I went with it. I don't think he's a butcher in any sense, but it wasn't what I was looking for either. He may have improved his techniques since, but I wouldn't know. Moral of this segment, when you think you've done all the research in the world, do more! Some people love a doctor and get a good result, others don't. Be very picky and make sure your aesthetic matches the surgeons you choose.
My 2nd revision was with a surgeon outside of the US, although he improved the appearance, I still wasn't satisfied.
Ok, now about the man who saved my nose! Heaven sent Dr. Eric M Joseph. My revision cost around $9,350. The surgical center was covered by my insurance, although I think it depends on which surgical center you go to. After much grief and loss of hope, seeing good and bad reviews about almost every surgeon, Rhino revision surgeons, whoever, I wanted to give up on getting my revision. I went back to my search for good results on Realself, MakeMeHeal, and found a girl with a similar situation as me. Third nose job, unsatisfactory previous surgeries . Judging from the pictures and her amazing story, it was enough for me to make a consult. Seriously, out of all the reviews, I'm lucky to have found her. Dr Joseph is super friendly and hilarious. He is very confident in what needs to be done, & clearly has much reason for it. I booked surgery during the consult, I was ready! The pre - op consult was also great, we did more simulations, I asked my million paranoid questions, showed him my CT scan, etc. He gave me Arnica to start a few days before surgery. I've never been given stuff from doctors, so this was cool to me. The surgery happened in a surgical center, all of the nurses were sweet and caring. I prefer surgical centers way much over hospitals, you don't have to deal with as much, it's less of a hassle, so it's a major plus in my eyes. The day of surgery was great, he marked me up and we had our final words about what was being done. Recovery sucked but wasn't horrible, no pain just some discomfort. I didn't need the painkillers. Breathing only out of your nose for a week always sucks. Key to recovery is to lay on a foam elevator triangle( got mine from makemeheal.com) & propped pillows underneath during recovery to keep the swelling down. I take it a step further with one of those neck U pillows you get at the airport, this way you don't accidentally smush your face while sleeping, don't want to risk your result! Yes, this is psychotic but the effective way of keeping your head still while sleeping. The aftercare package was amazing, tons of cotton, peroxide, snacks. The bag it came in was covered with nose drip pads taped all along the bag, so you could conveniently just rip one off every time the previous one got gross, no messing around with tape and gauze when your fresh outta surgery. Again, I lovvvveee the amount of extras you get with this doctor. The bandaging was cool & legit, he uses a metal piece to hold the bridge in place while you heal up. Bandages came off today, stitches hurt a bit coming out but eh gotta deal. Totally loving the result, my nose now looks straight as anything, it's shorter and more upturned, but JUST the right amount. This man's tip work is the best. He was big on making it skinnier, which I agreed, so it's pretty swollen all around. His aesthetic for what is an attractive nose is on point. He did an AMAZING job at giving me the cute, feminine nose I've been chasing for years now & fixed all the past surgeons work. It got even better when I was given a make up bag with 2 brushes, a color correction palette for bruising, chapstick, and bunch of other goodies. Yes, this sounds like the surgery/doctor fairy tale story but it kind of was for me, they are kind of the best surgical staff ever! I may be posting pictures when I heal up more. If you are looking to revise your nose or it's your first time doing a rhinoplasty, save yourself some time and $$$, consider Dr Eric M. Joseph. If you are from NJ or from the East Coast, or further, just make the trip. I traveled all over for results I didn't achieve and this time I traveled locally and got what I wanted...lol. Thanks for reading, hope this helped some of you. Feel free to reach out to me, I will do my best to do updates.

Consults
cottoncallsJanuary 7, 2015
I forgot to mention, I had consulted with Dr. Le in Baltimore for the revision. It was a good consult & I think he has an artistic eye for perfection, however I didn't know how I felt about using ear cartilage grafts. Dr Le likes to use ear/rib cartilage grafts, and I understand some cases may need them, but I feared the warping aspect that is possible with grafts years down the road. Go on a few consults to get different opinions, I was happy to hear Dr Joseph say the grafts were kind of unnecessary.

What kind of graft did Dr. J use for your nose? My surgeon used cartilage from my septum to support the tip so there wad no neec fot ear/rib cartilage.

Yea, I'm pretty sure that's what he did. He said he might use a spreader graft right before the surgery, but we discussed using septum cartilage. I don't think septum cartilage morphs because it's strong and it's what's already in your nose.

I know you need a post op, but I'm specifically thinking of going with him because he seems to have a few Indian patients and I just had a botched surgery with a septal cartilage shield graft that actually widened my tip without supporting my bridge at all.
Remind me--were you one of the people that went because of another revision patient? How did he make your tip "skinnier"?
A spreader graft is something tucked into the nose that can actually make it wider but its used for people that already have a lot of bony cartilage to work with.
If you have any specifics on what you went to him for (e.g., to shave a hump, break and reset bones) and what he did to improve you nose (used specific types of dorsal/tip grafts, redraped the skin), we'd be grateful. I can't speak for anyone else, but I'm seriously just considering an implant with ear cartilage to elevate my tip. I have a pretty short bridge, so I'm looking at tip grafts.

Congrats to you. Did he use Septal cartridge or what do you mean that he said grafts were unnecessary? Thanks for sharing with us.

Thanks love. He said using a foreign graft from ear or rib was unnecessary. Yea I'm pretty sure he used septal cartilage as a spreader graft they call it something. I have my post op consult in Feb, so I can ask him then. Regardless of what he did, it looks very cute and I'm thrilled =D

ETA: Please tell us what grafts he used and the type.
I am looking into a revision with him too. How was he able to narrow it?

I have my 2nd post op visit on Feb 3rd, so I will be making an update soon. I'm pretty it's septal grafts, he doesn't go for other types of grafts. he made it a lot thinner, still a lot of swelling but its going down day by day

OK, so here are my pictures. & to answer everyone's questions about grafts, Dr. Eric Joseph only uses your nasal septal cartilage to do the Osteotomy and lifting of the tip, pretty sure he called it a COLUMELLA STRUT. So, you're results are most promised for the future with out potential warping. Dr Joseph knows how to do a good nose! NOTE; Please check with your insurance company what the surgical center fee will be covered, because it really varies. I hope this helps a lot of people in need of rhino revision! I know how annoying it is to just want to have it how you want. Any additional questions just comment below.

Beautiful results. Great job!

I was going to consult Dr. J for my revision. Too bad I live far away. Thanks for posting photos.

Your nose looked so good before! Looks good now too, but imho not better, just different. I hope you can breathe out of that thing. I'm glad you got what you wanted.

Hi revisionhate, I looked again at cottoncalls' nose and she had it shorter and ever soooo slightly upward. Beautiful! Gorgeous! Just the nose I wanted and even I gave my surgeon model noses as this for, and....he did the exact opposite! What an ironic twist of destiny!!!

I agree, cottoncalls nose is much better after revision with Dr. Joseph. I considered Dr. J for my revision but it was a bit too far to travel. Great and skilled surgeon!! Maybe you should consult him, Silasolflamiredo.

How well are you breathing through your nostrils now (something I'm struggling with after having a big misused septal shield put in)?
Are you sure it isn't a septal shield graft (tombstone shape in the tip)?
I don't want to argue, but it usually says in post op notes what kind of graft he uses. A strut is just a straight thing in the columella, the "shield" is what actually projects it and is shaped kind of like a tombstone upside down.
Did he use the grafts you already had in the nose so you didn't need more cartilage removed? How bad did have your bones broken hurt? How was healing after the bones breaking?
I'm glad you're happy and hope it stays the way you like.

Hi anonpatient, what's "tombstone shape in the tip"? And "tombstone upside down"? I think that's what I had before my last 3rd surgery, but not sure. BTW I too am from NYC and after I search Dr. J a little I may go for consult with him. I live in Manhattan, and you?
February 8, 2015
@cottoncalls: another thing: I'm finding my graft really tight and uncomfortable. Can you smile and speak comfortably without feeling like your tip is dropping?

Congrats! Your nose looks beautiful. Its funny but my nose now looks so much like your nose before surgery and now im actually considering this doctor. Would you private message me?
