I'm so glad for you. Were your malar mounds/festoons congenital or something you entirely developed through aging? I have them genetically from birth and I'm just wondering what's in store for me in the future.

Congrats you look gorgeous! So glad this worked for you. Not sure you’ll ever see this, but did you ever get swelling in your eye bag or malar area, or is it just aging related? Yours doesn’t appear fluid related which might be why this approach was so successful.

Thanks for the compliment. Mine was full of fluid. My doctor when he saw me told me I had pools ther was so much fluid. Surgery was under local sedation, 15 minutes each side and they were gone. I still have small scars but was a great trade off.

Wow! Amazing result, and yes, I hear that’s usually the cause, and many surgeons say untreatable. This is very encouraging, and happy for you as I know first hand how irritating these are in making you look tired/sick to others. You really look well!

8 months out...thin scars but worth it. I use silicone scar bandages at night

Hmm, I really wonder if the scars have to be the same on each side or if they could be made more asymmetrical and "natural" looking, you know. But probably not. Is this photo with or without makeup?

The surgeon follows the lines of the crescent-shaped malar mound in my case it's Improvement not perfection and it takes up to a year for a scar to heal but if you put the silicone gel on it vitamin E it you just have to count the cost to you to me it was a hundred percent worth

Yeah no kidding, your improvement is amazing. And your scars are still fresh. You could also improve scars more with microneedling or lasers at a later point. Even do micropigmentation to blend it with your skin tone if it turns into a hypopigmentated scar.

I am doing follow up laser. I wish you well. I had seen at least 7 different surgeons, all with different approaches...i knew direct excision was the only real choice for me.

Yes, definitely better with scars than those malar bags. I can't believe how small he made your scars, they are barely noticeable and look nothing like what I had expected. It's really absurd though that we've come so far and yet the only way to get rid of malar bags is through direct excision. It's frustrating, but I'm going the excision route too when I get older. I think I'm too young right now for excision, as they're not as developed yet as yours.

Be careful with the laser though, as heat in this area can actually cause malar bags, so find someone aware of this. I'd honestly try microneedling first since it's so safe and from the published data pretty much equally effective to laser, believe it or not.

The surgeon who did my direct excision is also the one that does my laser.. yes it is quite surprising that the only way they can get rid of malar bags and festoons is through direct excision but I'm thankful for it probably in 3 or 4 years I'll have a different way

Hello Lucy, I am a 68 yo male with malar festoons and am just beginning research for treatment. I understand the direct excision surgery, but did your doctor recommend this over laser? Or, can the scars be revised with laser later? You look great now and I am so frustrated with festoons, I have had them since college.

I went to 6 different surgeons, some said it's not fixable, some said deep laser but many who had laser said the festoons returned. From my reading and the extent of mine I knew direct excision was the choice for me. Mine were full of fluid, when Dr.Balikian saw them he said direct excision and I knew he was right.

I wonder, have you always had malar mounds since childhood, and then they turned into festoons at a later age? I'm too young for festoons but I have malar bags, very prominent, and I wonder what the future holds for me..

No...they have shown up in the past ten years and really bad the past three years. I took my time finding the right person to help. Dr. Balikian in Murrieta, Ca is wonderful. I his assistant has them too but not as bad as mine, He treated hers with laser and filler. Her name is Noelle...worth a call or email to her.
