I am just adding a before and after picture of my back to show the change after surgery. I do not like how it looks now and really liked my lower back before. Though the main issue here is the discomfort.

ORIGINAL POST

Dimples, Irregularities and Sagging.

I am very upset with my results after surgery with Dr Kohout in March 2026. My upper abdomen is covered in dimples and irregularities, my figure has changed significantly, and I believe liposuction was performed on areas I did not ask to have treated. I am 30, have never been pregnant, and my BMI has always been in the healthy range. Before surgery I was largely happy with my body and only wanted small improvements. I am now quite uncomfortable and self-conscious about how I look, and I have ongoing physical pain that affects my work, my social life and activities I used to enjoy.

I contacted Dr Kohout in writing and had two consultations, one in person and one over Zoom, where I showed photos and an annotated picture of myself so my goals were clear. I wanted to improve the “banana roll” area under my bottom, mild skin laxity on my upper abdomen, and laxity in my breasts. We agreed on liposuction of the banana roll area with the fat transferred to my breasts, and BodyTite of the upper abdomen. As there wouldn’t be much fat from that area alone, we agreed to include my flanks. I said I would be happy with even a small improvement to any of these areas.

As the swelling settled, I found my outer thigh/buttock area and lower back had also been treated, and very little fat remained in these areas. We did not discuss either area at my consultations. I understood the posterior thigh liposuction would need to be blended into the surrounding area, but I did not want the shape of my outer thighs or buttocks changed, and I had shown photos of outer thigh results I did not want.

I sent photos of my abdomen to the practice within the first few weeks of surgery. I was told it would improve with ultrasound massage, which made no difference.

My results seven months on:

• Upper abdomen: Dimples, indentations and surface irregularities across the whole area, visible since early in my recovery. Before surgery I only had mild skin laxity here.

• Lower back: Almost no soft tissue is left over my lower back, sacrum and tailbone, and I can feel the bones directly under the skin. My Venus dimples are gone, the area looks unnatural to me, and there is a roll above it. It is painful sitting in some chairs, and to lie down or stand for long periods.

• Outer thigh/buttock area: Very little fat cover remains, and I can feel the bones here too at the top outer thigh/buttock area.

• Body shape: Before surgery my upper thighs and bottom were the widest part of my body (which I liked). Now my hips and flanks are the widest part, giving me a much squarer shape. The lower part of each flank is still there, which looks strange like a separate lump on each side.

• Banana roll area: Looks saggy now.

• Breasts: I don’t believe any of the transferred fat was retained. They did swell very very significantly after surgery though, so now I have more laxity than before.

Overall, I feel I look worse, older and more masculine. With so little tissue left over my tailbone and lower back, I feel pressure directly on the bone when I lie on a hard surface or sit in certain chairs. I used to do yoga every day, but I now can’t lie on a mat without padding underneath, and my movement is more limited. I also enjoyed hiking regularly but the painful pressure on my lower back from a hiking pack makes this very difficult now. Alongside the physical discomfort, I feel self-conscious about my appearance, which affects my work and social life.

I’m sharing this so others can make an informed decision.
