Went to our dentist two years ago and was told it would cost +/- $100K and be a 1-2 year process to replace my teeth. So I decided to wait until something actually happened to proceed. Well between losing an implant recently, my 36 year old bridge starting to hurt (still had the nerves in the bridge support teeth), loose bottom teeth and two implants into my right sinus causing my right nostril to bleed a lot lately, I knew it was finally time to do something about my teeth. Found a local dentist who had a lot of experience doing implants, but his prices were even worse, so I started expanding my research area to nationwide and found Golpa Dental Implant Center. Dr Golpa and the G4 implants met my criteria, so I made an appt.

Three weeks ago we flew to Las Vegas. I had my consultation on Tues, surgery on Weds, the G4 bridges placed on Thur and flew home on Fri.

For most people they have very little pain after surgery, but I had extensive work, so while the surgery itself was pain free due to the anesthesia, I’ve had pain or aching ever since, but it’s getting better everyday. I wake up to no pain, but by nighttime, my mouth is aching. The prescription for a large number of hydrocodone pills was a clue I would not have a pain free process.

They extracted 15 teeth and 3 implants, performed a sinus lift and bone graft where the previous implants were into my sinus cavity, performed alveoloplasty on both upper and lower jaws and placed 9 implants; 4 implants on the bottom and 5 implants on the top, with the rightmost upper implant hidden under my gums for 6-12 months depending on how I heal.

I go back in 6 months, unless I have problems and need to see them earlier. I was instructed to be on a soft diet for at least three months and was told to chew on my left side only. My lower bridge has 12 teeth and my upper bridge has 10 teeth. There are no teeth over the area when the sinus lift/bone graft/hidden implant is located. I’m using the Waterpik several times a day, but still too swollen to floss.

No pictures because I’m not really happy with my smile and will address that when I go back for a followup.

Golpa Dental is running a special to get other dentists trained in the G4 implant procedure. I knew I had a complicated mouth so I requested Dr Golpa do my surgery rather than the trainee. For that I paid $5000 extra. Also paid $3000 extra for 3 implants to be removed and $4500 extra for the sinus lift and bone graft. Total cost was $44,000 for the surgery and another $3500 for the extended warranty for a total price of $47,500.

I have been contemplating going to Golpa and need 19 teeth extracted and most likely a bone graft and sinus lift. I have also been looking in Mexico and Costa Rica. I know there are really good quality dentist there. It will cost me the same amount to fly to Yuma Az (MX) as it will to fly to las Vegas. The cost in Mexico is around $17,000 and they pay for your hotel and travel to and from the airport and hotel.

I know it's a tough decision to make. I have several friends who went out of the country to Mexico, Costa Rica and even Croatia for dental implants and they are all pleased with the results. I thought about it, but decided to have the work done in the US.

Thats expensive. I also need extensive work n was told complex cases start at 42500. Plus sinus lift 4000 plus removal of 6 implants 6000 price 52000.
I was under the impression g4 implants were for people with dentures n extensive bone loss.
I am disappointed

The advertised cheaper price is when the drs in training perform the work with Dr. Golpa supervising. Dr. Golpa does the complex cases rather than the drs in training so the price is rightfully so higher and you really want someone with extensive experience doing the surgery if you have a complex case. If your case is complex, you’ll likely be quoted a higher price everywhere.

Thanks. No, I paid extra for three 20+ year old implants to be removed, a sinus lift, bone graft and for requesting Dr. Golpa to do my surgery, because I already knew my case was complex. Sometimes they run specials. You can look on their website for the current price of a typical case.

I've decided to post before and after photos of my smile. Not sure you can tell from my review, but I am glad I went to Las Vegas to have the G4 procedure. I just think my smile can be improved considerably. There's also a tooth on the bottom bridge that's more pointed on the sides and is whiter than all the rest. It doesn't show up in photos, but it's quite noticeable in person.

The area where the sinus lift was done was more tender and sore longer than other areas of my mouth, but you’re given plenty of pain pills, if needed.

Dr. Golpa removed two implants that were either placed into my sinus cavity in 1995 or worked their way into it over the past 22 years. Dental implants should not be in your sinus cavity and I’ve been lucky in that they didn’t cause me any issues until approximately a year ago when they started causing nose bleeds. After removing the two implants, Dr. Golpa performed a sinus lift, placed a new implant and also did a bone graft, since I needed bone in that area. He didn’t attach the implant to the titanium bar, because new bone needed to form for six months in my case to hold the new implant in place.

No, the bridge would not be strong enough, if they extended the titanium bar to add two teeth. They will make a new bridge and I actually received it yesterday. I now have a new bridge and am quite pleased with the outcome and will post a full update in a few days.

6th Month Follow-up

I returned to Las Vegas this past week for my 6th month follow-up and to attach my hidden 5th upper implant to the bar if my bone had fused enough to the implant. I discussed with them my concerns about my bridges. Removing the upper bridge was easy, but the bottom bridge was partially destroyed, trying to loosen one of the screws. I was really concerned about them torquing my implant, which has happened to me in the 90s. The tech tried, another dr tried and then Dr. Golpa eventually got the screw unloose without damaging the implant. They uncovered my 5th implant with just a local (my choice), made impressions, cleaned my old bridges, and reattached them until the next day. The next afternoon I returned and they placed the new bridges into my mouth and told me I can now eat anything I want. Yay! I’ve really put my mini food processor to the test these past six months and I’m so looking forward to eating nuts, pop corn, dried chewy fruit, etc again.
Now that I’m finished, except for 6th month follow-ups, here’s some additional thoughts on my G4 experience.
After looking at their website, other websites and watching all their videos, I was really excited to know I would have teeth in just a few days, would have no pain, nor need a sinus lift or bone graft and would be able to eat normally in only a short time. Realistically I had a hard time believing there would be no pain, since I had previously had six dental implants procedures done and remembered the pain, even 20+ years later. As far as eating normally a few weeks after the procedure and no sinus lifts/bone grafts would be needed, I had no reason to doubt what was written and perhaps it is the norm, but in my case it was untrue.
I do believe some people may not have much pain after the procedure, but I also think some of the videos are done while their nerves are still deadened and before the swelling and bruising begins.  I know I read a comment on one of Kalinjax's videos where a guy says his wife did a video saying she was in no pain and then later that night she was in severe pain. I can tell you I had pain starting the night I had surgery for several weeks, which seemed to be worse at night, but even with the extensive work I had done, the pain the first week wasn't as bad as my six dental implant procedures I had done in the 90s and for that I’m very thankful.
As for eating, because of my poor bone structure, I was told when they eventually followed up with a phone call almost a month later, that I needed to stay on a quasi-soft(cut with a fork) diet not only for three months, but until I returned to their office in six months.  
As for the G4 process not needing sinus lifts and bone grafts, I know the day I had surgery, Dr Golpa told me he was doing sinus lifts and bone grafts on five other people that day besides me. Speaking of bone grafts, they won’t be able to tell you what the additional cost will be for the bone graft material until they know how much material will be used.
So if you’re looking to have the All on 4 procedure done, just be aware that we’re all different and your experience may be different than someone’s you’ve read on the internet.
Our teeth affect our health and I should have done this several years ago, instead of postponing it so long. I postponed it not for money, but because I dreaded the two year process I was told it would take to get a decent smile. Also until I found Dr. Golpa’s website, I didn’t think the All in 4 procedure was right for me. Once I saw the G4 process and the titanium bar which spread out the force over the entire mouth, instead of just the implants and saw Dr. Golpa’s extensive implant experience, I knew I had found the right person for the job and I was right. Best thing I’ve done in a long time!

My original G4 bridges were what I call ‘gappy’. Too much gap between each tooth. I would have never approved that teeth option had I been given the choice. FAQs say you get to choose. No you really don’t. You discuss what you like and dislike, but aren’t really shown different smile options. With gaps between the teeth, the unpolished sides of the teeth were visible. I drink coffee all day long and the coffee stained my teeth. I called them and asked what to do and they told me no one had ever complained about coffee stains and didn’t have a solution for me to remove the stains. The second issue was the centerline was off. The third issue was the bridges were too far left with too much space on the right. The fourth issue was with the shape of my mouth one of the screws ended up in the front rather than the back. The mass I was describing in an earlier post was actually composite material from filling the screw hole. Of course I didn’t want that visible, if at all possible. With the teeth shifting to the right, the screw hole would end up behind one of the teeth, instead of in the middle. With all these issues they agreed to remake the bridges. The top had to be remade anyway for the 5th implant to be attached and the bottoms were damaged while removing one of the screws so even without any issues, both would have been remade. Since both bridges had to be remade, I chose a shade lighter.

My new smile is not perfect, but it’s much, much better. I have a tiny/child size mouth and the teeth are larger than they should be, but larger is better in my opinion than gappy.

When I looked at your before and after from April, I thought the after looked great but it is hard to tell from a single picture. I think my teeth are too big in the front as well. I plan to address that at the end of this month. My bottoms are bring remade and even though the bite feels great, I have an overbite that I dont like at all which makes them look uneven with the top. I'll hopefully have that taken care of as well. I got the middle shade of white and it seems very "white" to me. I guess I can live with that. I only drink 1 cup of coffee a day and so far no staining. I brush 3 to 4 times per day. I feel like until I get this smile perfect the way I want it that I really cant enjoy taking pictures yet. Hopefully I will by beginning of November :)

On my 1st G4 bridge, look at the shape of the teeth. They're much narrower at the top than the bottom. They did tell me they were going to get rid of my overbite and they did some when he put in the implants, but I still have an overbite. Perhaps that's on purpose. I do know it's much better than I've had my whole life, so I'll take it. For the 2nd G4 bridge, I believe they told me the new shade is B1. If none of your tooth surfaces are rough, then you won't see coffee staining. It's like they didn't polish the sides of the teeth on my first G4 bridges. On the smooth surfaces I had no problems. I totally understand about not smiling. I've never smiled so little in 6 months than before I received my original porcelain bridge in 1981. Now I can smile again.

If you're still not 100% satisfied, have you met them know exactly what you want? Maybe bring in pictures? Have they said if there is a reason you can't get exactly what you had in mind and wanted. It's hard to tell from the little picture you posted. My issue isn't so much the teeth but it is the placement of them. My whole upper lip and bottom part of my face is all wribg and very drawn. I haven't seen too many people with that issue when i scroll through the photos.

I was just there this past week and discussed exactly what I wanted with the assistant and José. Like I mentioned earlier I have a child size mouth. Dentists use a pediatric impression mold on me when they take impressions. I think my choices are larger teeth or teeth with gaps. I'll take the larger teeth over teeth with gaps any day. I'll discuss it with José again when I return in 6 months. I do know my appt. was postponed twice on the day I was to get the new bridges, because they were having issues with making them.

There was only one point of contact on a left molar between upper and lower. He shortened that molar until I touched one other molar on the right. After a few days I was touching only on the right side. He shortened that one at the one month visit. Nothing else was done. I still bite my chick at the point of contact. The centers of the bridges are not aligned, the upper is placed higher on the right and lower on the left, smile is deformed.

The placement is my problem too. Did you try to get new temporaries placed correctly? I asked but I was told that I have to wait the 6 month, and then the wax-try-on will be done correctly and redone if necessary until I am satisfied.

That's good that you are documenting everything. I did not have any temporaries. I only went back for my recare visits, which for several years was every six months. At my 6 month visit my bridges had to be remade, because they were uncovering my 5th upper implant to attach to the bridge. The material had changed so the bottom bridge was remade too. From what I have noticed over the past 5+ years reading lots of posts, bites are trickier with small mouths. I'm not sure what recare visit it was, but I finally convinced them to shorten my teeth a bit and it helped to not bite my lip, but even today I occasionally bite my cheek, but thankfully it's not often. I really wasn't happy with my smile until I got my nano-ceramic bridges at the Virginia office.

Prices change over the years and it's best to look on the G4 by Golpa website for their most current prices. Complicated cases like mine do cost more.
