I have been a member of RealSelf for a while now and I am very excited to be finally sharing my own story with you. I am a female of Middle Eastern descent and I have always been very unhappy with the size of my nose. I believe that a primary rhinoplasty would restore my confidence and give me the well balanced facial structure I have always wished for. Here is a list of the things I don't particularly like about my nose:
- large size (most problematic aspect of my nose)
- hanging columella
- slight dorsal hump
- long and asymmetrical nostrils
- deviated septum
- boxy tip
- slightly wide nasal bridge

I have been searching for the right time and the right surgeon for a long time. Being an undergraduate student has made it quite challenging for me to arrange some time that would be appropriate for my procedure. It is my last year of studies and I think it is a good chance for me to schedule my rhinoplasty in the upcoming summer. My search began with surgeons in my hometown, Toronto, ON. I had my first and only consultation with Dr. Oakley Smith last year. I had read amazing reviews about him and was determined that he would be my surgeon. However, after meeting with him, there was some internal instinct that held me back from booking my surgery with him. My boyfriend who was at the appointment with me thought very highly of Dr. Oakley Smith but I was convinced that he was not the right surgeon. Dr. Oakley has had many happy patients but based on his suggestions, I believe that he will not be able to make the changes I would like. I realize that a perfect nose can't be created through rhinoplasty and I have reasonable expectations but I would like my nose to be as close to the ideal perception that I have.
Here is a list of what Dr. Oakley had suggested to me:

1. Remove hump. (95% of the time this degree of change occurs.)
2. De-project tip by 4mm. (90%)
3. Decrease overall size of the nose. (90%)
4. Soften crease on tip so it is no longer boxy looking. (80%)
5. Raise the flesh at the bottom of the nose slightly. (95%)
6. Rotate tip upwards slightly. (90%)
7. Have a significant but natural look.

I accidentally came across Dr. Grigoryants' (Glendale, CA, US) photos on RealSelf and was amazed by his work. In my opinion, he creates the perfect nose! He seems very talented in what he does and he has many pictures to prove it. I have had email conversations with him and he has suggested the following:

" I would suggest removing the bump, shortening the nose, narrowing the bones, refining the tip.
I may need to shorten your nostrils as well I will open your airways at the same time if necessary."

Dr. Grigoryant focuses on closed rhinoplasty. I have a slightly deviated septum and require tip reshaping. Can these complications be corrected through closed rhinoplasty? I agree with everything he has mentioned and at this point the only hesitation I have is the need to travel overseas for surgery. Do any of you have any experiences with going abroad for rhinoplasty? What are some of the complications? Is it worth the risk?

Ahh!! You are in Toronto as well :) I am also considering doctor Grigoryants. I am so excited to see your results! I'll PM you and we can chat since we are in the same boat. I was looking into Oakley/Rival/Solomon. Booked with Epstein in Miami but then cancelled. Now I am considering Dr. Grigoryants. I think it is worth travelling to get the best results possible!!

Do any of you have any experiences with going abroad for rhinoplasty? NO EXPERIENCE BUT I THINK THATS THE BEST BET. BASED ON MY RESEARCH YOU SHOULD HAVE SOMEONE GO WITH YOU TO TAKE CARE OF YOU. What are some of the complications? Is it worth the risk? YOU CAN GOOGLE THIS. OTHER PEOPLE CAN GIVE YOU INSIGHT. ONE OF MY BIGGEST CONCERNS IS SCAR TISSUE AND HAVING AN UNSTABLE NOSE.

You're so pretty so I can only imagine how amazing you'll look/feel after you do this! I'm also 22 and Middle Eastern-Canadian. At 19 days post I'm already so happy I did it. Mine was an open septorhinoplasty and I'm glad he opened it just because I personally think a surgeon has more control this way; however I've read amazing things about Dr. G. I was going to go abroad for my rhinoplasty but decided to do it in my hometown just because it was the easier option (being home, able to see the doctor whenever, comfortable during the fall/winter, etc). Mind you, many of my friends did theirs overseas (in Lebanon) and I only know of 1 experiencing complications. I had also considered going to Toronto for mine just because it seemed there were so many skilled facial plastic surgeons so it seems you have many options! I just found myself more at ease, knowing I could go back to my surgeon at any time (ie. with any complications). I will be seeing mine again in a few weeks and he will continue evaluating it throughout the months to come. I'm sure you know that aside from seeing your surgeon's work and trusting his/her ability, the most important thing is that you communicate what you do and don't like. Providing him with a visual of your ideal outcome is great but try not to get too fixated on the before and after photo you've posted. She seems to have a similar nose but all cases are different. It's great that you're being realistic and realize there's no such thing as the perfect nose. I'm sure that you will be pleased with any skilled surgeon. Have you consulted with any others? Actually, most doctors who are skilled enough to do closed rhinoplasties say that it is much better than open rhinos for many reasons. One, when the nose is done in an open fashion, many grafts have to be put in to stabilize the effects of opening the nose. Skilled doctors can achieve better results that most doctors doing open. The issue is that most doctors don't choose to learn the closed the technique these days, because it is much harder. Someone else here posted something that was posted on MakeMeHeal re: open vs closed. I don't think we are allowed to link, but if you look under Ms.J "open vs closed" there is quite a bit of good information.

That's interesting. The surgeons I consulted with do both closed and open depending on the complexity of the case, both functionally and cosmetically. A friend of mine has had two closed rhinoplasties done and her nose is far worse than before her primary rhinoplasty. I guess she's the reason why I assumed some cases should be done open, as I've seen other patients of his that turned out fine (using the closed technique). I will check out the post you've mentioned, although I've already had my surgery and am happy with it. I personally think I needed far too much tip reconstruction for a successful closed rhino. It needed grafts regardless but maybe yours is simpler. I had an excessive amount of cartilage forcing my droop. Good luck again :)

It makes perfect sense but the only thing I am concerned with is my deviated septum which may need extra grafts (don't know if closed is the best option for this). Did you have your procedure done by Dr. G? I wasn't able to find your review. How was your experience? Did you consult other doctors? -waitingmywholelife

Hi finallyformyself, Its so interesting how we have so much in common. I have also wanted a rhinoplasty as long as I can remember. I am glad you have had a positive experince with your surgeon. I consulted only one doctor in Toronto (the one with the most positive reviews) and I just couldn't convince myself to go ahead with the surgery. It seems like there a lot of options in Toronto but once you start reading about them and going onto their websites, your choices get narrowed down very quickly.... there are tons of negative reviews about some of them unfortunately. I could be just looking into it too much but I have to trust my instincts. I definitely agree with you that having a surgeon in your hometown is much easier! I am really hesitant to go to the US but that seems like the best option for me at this point. I can relate to how you felt before your rhinoplasty... Regardless of what your family members, friends and your loved ones say, if you don't feel good about your self, you won't be happy about the way you look. And thats exactly how I feel. Us Middle Eastern girls have not had the best luck with noses... I have my dad to thank for mine. I am very insecure about my nose and I am excited to be finally going ahead with this procedure. You are so pretty already I wouldn't waste your money or time on getting anything done. You are not guaranteed the outcome you think. I had rhinoplasty surgery done 5 months ago for minor changes and have regretted it ever since it is not how I thought it would be and I wish I had May old nose back everyday. This is the centre of your face and once it's done you can never go back. Please don't do it.

Dear Sarz, I just read your story and I want to wish you all the best. I am so sorry that you have had such a negative experience with your procedure. Your story was very eye opening. To be honest, I have waited for so long to get my rhinoplasty because of the fear of having an unsuccessful procedure. I can just imagine the toll this experience has had on you and your family. Please don't lose hope in all surgeons and know that there are always good and bad ones. Although you may feel so negatively of rhinoplasties at this point, I hope you will be able to go ahead with a revision in the future to improve your breathing problems at least. I went on Dr. R's website and right away noticed that he has linked RealSelf on his homepage ("read our reviews on RealSelf"). I find that odd.. considering there are negative reviews of him online. Also, he doesn't have any before and after procedures... that raises red flags in my opinion. Did you consult any other doctors before your procedures? Were there any red flags? Any things I should look for when consulting my doctors? Thanks for your comment and concern. Your story reminds many of us on RealSelf to be more careful in making decisions about our rhinoplasty. No I only consulted one surgeon who's before and after photos looked good and I could only find positive comments about him. I now realise I was unhappy in my life and wanted a rhinoplasty to feel better about my self. I really would not get anything done if I was you even the best surgeons can get it wrong and everybody heals differently. I use to fantasise about having a beautiful smooth nose mine was similar to yours side profile. But now I have it it doesn't suit my face. When I smile my cheeks look weird because the bridge is narrower. What people don't realise is that even minor altering of the nose can change the way your eyes look when you smile your cheeks your lips everything! They can't show in the photos how your skin will swell where it will swell and how how other features will change with the new look. It is only a guesstimate. Spend your money on seeing a psychologist who'll will help you love you for you and forget about your nose. Think about the people who love you and I can assure you they love you just the way you look now.

Other doctors in LA...?

Hello Everyone,

I am considering other doctors in LA now. I have come across Dr. Ghavami. He seems to have positive reviews and his website is filled with before and after photos. I have communicated with his receptionist over email - he has a $250 Skype consultation fee. I don't know if it is worth the time and money to consult with him before making my decision. Does anyone know about any unsuccessful stories of Dr. Ghavami?
I am hesitant to book a procedure with Dr. Grigoryants before having the opportunity to meet with him over Skype or in person. Is it a bad idea to book a surgery before meeting with your surgeon in person?

Contrary to televisions version of quick and easy cosmetic surgery; I've learned through experience and research that nose surgery is rarely quick or easy. By all means, do see your PS in person before booking a surgery. I am currently searching for a PS to do what I hope will be my final revision. From my past experience, I've learned: 1. Research, research, research. Learn as much as you can about the procedure, the healing process and emotional/psychological impact 2. Always meet with your PS in person before booking surgery (if possible). I've had a couple of Skype and email/photo consultation with drs; then followed up with an in-person meeting. Both times the Dr's assessment of what could/should be done to improvement my nose changed after seeing me in person. 3. Don't pick a PS based on fees. After multiple surgeries on my nose, I can personally attest that it is wise to go with the surgeon who not only can demonstrate good results but one who has experience addressing the results that you want to achieve and has work with patience with your skin type (thicker skin can be more challenging). Wait and save if you find a dr that meets your needs but you can't immediately afford it. You won't regret it. 4. Remember that we all heal at different rates. It took almost 18 months to see the final results from my first surgery. But it only took 6 months for my good friend to heal even though we had the same type of work done on our noses! 5. Be proactive in deciding what you want done. If a dr suggests a change that you don't want or like, don't be shy about disagreeing with him/her. It could be that what you desire can be done but s/he doesn't think it will suit you. 6. Prepare your mind and body for before and after surgery. Follow the Dr's orders precisely even if you think you are up to bending the rules a bit... don't. Good luck and blessings to you in your search for the right surgeon. I'm sure you will make a good choice and have good results. Keep us posted.

Blessedmore2014 has given you great advice. I would meet with your surgeon before deciding for sure if he/she is the one to perform your septorhinoplasty!! It's hard to tell completely from pictures or skype. The surgeons I emailed didn't even mention my deviated septum, they just described it as a slight nostril assymetry and said they would do it closed. Then when consulting with 2 surgeons in person they told me I had a deviated septum that would be best corrected with an open procedure, then mentioned way more to me than those I connected with online or over the phone (with pictures). Also, my surgeon was an ENT specialist along with facial plastic surgeon which was important to me because breathing is such an important factor. Looking great is one thing but having limited nasal airways is another. Can't wait to read more about your journey :)

medical insurance for overseas procedures?

Hi Everyone,
I am wondering if I will need to purchase additional medical insurance when going overseas to get my procedure? I have OHIP (government-run health insurance plan) in Canada but I am not sure if any emergency/unexpected complications as a result of a medical procedure (hopefully won't be the case! but you never know...) will be covered. Has anyone gotten anything similar or thought about this? Any advice or suggestions would be very helpful.

Also, has anyone had an online consultation with Dr. Grigoryants? I would like that before booking.

I purchased a travel medical insurance from RBC. If there is an emergency then it should cover the expenses. Crossing my fingers that nothing like that is going to happen though.
