Bulky Ear Cartilage Grafts Gone and No More Eyelid Webbing

WORTH IT
Below are my prior surgeries and what initially started my long journey. The reason I had surgery with Dr. Guy Massry and Dr. Raymond Douglas was to correct round eyes with outer webbing and I really hated the bulky ear cartilage in my lower lids. I've very pleased with the results. I do have some asymmetry and slight retraction in my left eye but have no regrets about this surgery. It was totally worth it.
December 2004
Went to a Plastic Surgeon
Surgery Upper and Lower Blephoplasty with the incision on the outside of my lower eyelids. (Ask me why 95 percent of the time you should not get cut under the eye on the outside)
February 2005
Went to an Ocular Facial Plastic Surgeon
Surgery Canthopexy on both eyes (Ask me why 95 percent of the time you should not get a canthopexy to treat lower eyelid retractions)
Reason: Eyelid retraction
January 2010
Went to an Ocular Facial Plastic Surgeon
Surgery Thread lift to hold cheeks in place, ear cartilage grafts in the lower eyelids, and canthoplasty. (Ask me why a Thread lift is not a good idea and why to never get ear cartilage as an under the eye graft)
Reason: Eyelid retraction
November 2017
Went an Ocular Facial Plastic Surgeon - same Doctor as the last
Surgery Lateral canthal lengthening on right eye with 1 tarsal bite from the periosteum
Reason: Right outer corner eye bump and corner slightly pulling apart with webbing.
December 4, 2020 I had surgery with both doctors below.
Dr. Guy Massry & Dr. Raymond Douglas - LA
Dr Douglas performed orbital decompression. By pushing the eyelids back
it lengthen the canthal tendon and provided more skin under the eyes, which I needed. Dr. Massry then removed my spacer grafts, performed a mid face lift, lengthen canthal tendons and corrected outer corners of my eyes.

Hopefully it will last for years.

Wow, you look great (and happy!) Thanks for sharing your story and giving all of us with retraction some kind of hope. So Dr Massry removed your spacer graft and replaced them with what?

Spacer grafts were not replaced at all. With my eyes being moved back (orbital decompression) and the mid face lift Dr. Massry felt he didn't need to replace them. This was decided during the surgery. Dr Massry's original plan was to replace the grafts with a hard palate graft but told me prior to the surgery he might not need to replace them at all. So, during the surgery he made that decision not to replace. I do have slight retraction in the left eye but not any worse than I had prior and my guess would be for Massry to remove the grafts and replace them during the same surgery along with the orbital decompression and mid facelift would have been quite and undertaking. During my consult he said he didn't know what exactly to expect after removing the grafts. There's risk due to many nerves in this area. I think I was clear I disliked the grafts so much that I wanted them removed.
I had a consultation with another prominent Ocular facial plastic surgeon from Beverly Hills and there was a plan to fixed my issues, but it didn't include graft removal. And prior to the plan of having Massry and Douglas work together I had a consult with Dr Douglas he felt it was pretty risky to remove the grafts.

I experienced lower eyelid retraction after doing transcutaneous lower blepharoplasty last year, I have consulted several clinics in Korea and they advised me to do canthopexy, but after reading your review, I was worried about doing the procedure. will it make it worse? I'm also planning to do a thread lift to lift my cheeks? why don't you also suggest a cheek lift? I need your advice so I don't make the wrong decision

First off I'm not a Doctor so take my words with a grain of salt. In my experience a canthopexy did not work for me. When you have lower lid retraction and you tighten the canthal tendon that doesn't mean it's going to stay raised. I would say most cases there needs to be skin added to the under eye area to support that tendon by using spacer grafts, a midface lift, orbital decompression, orbital rim implants or any combination of these. You need to post photos and ask doctors on realself what they may recommend. In regards to a thread lift I had this and it didn't last. I think the manufacturer of the threads is on their 4th new and improved thread. A thread lift will last a year if you are lucky.

Your smiling eyes are giving me courage. Thank you so much for writing your experience and what you have gone thru as well as where you are at now. I was considering a canthopexy to fix problems that are very similar to your before photos because it was somewhat affordable, though my gut told me that it was just too good to be true. My original surgeon won't touch my eyes and tells me there is nothing wrong. I have tried to hide behind my glasses for two years and the emotional toll is great. I'm going to do everything I can to save up for this procedure and just wish that my results will be even close to yours. Just fantastic and thank you again.
