Office review - resistant in providing me the information I needed

 I spent my entire session speaking with the Assistant of the doctor, and the doctor came in and spoke to me for about 5 mins to inject the ExoFlo. ExoFlo as explained by the doctor are stem cell injections intended to create new hair growth. The physician gets a 1 star because of my experience after the treatement/service was provided. My insurance provided reimbursement for procedures/treatments conducted by out-of-network doctors and when I needed certain information of the physician the office was resistant in providing me the information. When it came time to ask for the physician's Tax ID # and NPI # so I could file a claim with my own insurance to be reimbursed, the staff members and doctor were not pleasant, or willing to provide me the information. I went back and forth with the receptionist, to then be transferred to the director of patient care (Jonathan is his name). When I asked for the information he needed to "verify" with the doctor first. The doctor confirmed the CPT code for the type of service I received but provided inaccurate TAX ID and NPI number. My insurance just told me that the information provided was not accurate at all. I had to google this physicians information and look her up in on uniprofile. I wouldn't recommend this office at all anymore.
