Migraines, Neck, Upper Back and Shoulder Pain

At the age of 15 and 112 pounds when all my pain started. I suffered from headaches, neck, back and shoulder pain daily. I also had sagging breast. I am now 42 and 190 pounds and over the years had several physical therapy sessions. I just finished another 6 weeks last week. It has never helped. I've been prescribed different types of pain medications. I have had several injections in my back by different pain specialists and Chinese cupping which none of it helped my pain at all. I wear a size 40 DDD bra. My breast hang very long. I can't see my nipples. I buy those 18 hour bras and wide straped bras that is supposed to give me comfort. But, after having my bra on for an hour or 2 I want to pull it off. After I take my bra off it feels like a big relief until 45 minutes later and all the pain is back, intense migraines, neck, upper back and shoulder pain. I try to pay attention to how I sit so my posture is straight. But, before I realize it I'm back hunched over. My breast has always been very sore. So I never liked them touched. Feel like needles is prickling me. My breast get in the way of me working and exercising. Years ago I had a back doctor to tell me I needed a breast reduction. I wasn't ready then still in my early 30's. About two years ago my doctor sent me to a plastic surgeon to see if I qualified for a breast reduction. His response was that although my pain is due to my sagging breast that I didn't have enough tissue to be removed in order for my insurance to pay for it. I knew I couldn't afford to pay for it so I figured I would have to live with this pain for the rest of my life. But, this past June my pain came with a vengeance and not even for a minute has it let up. I have knots on my shoulders where my muscles has spasmed up and been that way every since. I went to urgent care I was in so much pain. The doctor there told me to stop working because I would never get any relief. Well, I can't completely stop working so I cut down to 4 days a week. I called my insurance two different times to talk to different people and asked the same question is there a certain criteria I need to meet in order to have a reduction. They both said no. I need a referral from a back doctor. Then the insurance would figure it out from there. My question is with everything that I have mentioned do you think it will be hard to get the insurance to approve my breast reduction?
