Looking to Downsize Breast Implants from 375 Cc Silicone to 275-300cc

I had a breast augmentation 9 months ago (I was a 32 A). I had it clear that I wanted a natural look and showed my surgeon my "wish boobs" (that girl got 275cc silicone) I told this to my surgeon but he said there was no way her boobs were that size, so he said it would be ideal to get between 350 cc and 375 cc. I trusted him and 9 months later I still can't get used to them. What bothers me the most is the projection. They look too fake, they're way too big.
I really want to downsize my implants and I want to do it as soon as possible. What would they look after doing that? My skin is young (I'm 26, 112 lbs and 5'4). Would I see a difference by reducing them by 100 cc? Would the revision be worth it? Also I want to get a scar revision (half areola) at the same time because I have developed thick scar tissue and it hurts to the touch. I would appreciate it your feedback :)

Hi csantam, what implants do you have and what is the profile? Were your implants placed over the muscle or under the muscle? If I were to guess, it looks like you have high profile or ultra high profile implants which give the "fake" look that some women desire. High profile implants allow thin women with very little breast tissue to get more projection because they are smaller in diameter in relation to their volume. Because the communication was so poor between you and your PS, I would suggest you get a second opinion prior to trying to get it right with your current PS. It can't hurt to get a second opinion. Can you post a picture of your "wish boobs" along with the manufacturer, model and profile of your implants? The "wish pictures" (which define what you feel is a "natural" look" - remember everyone has a different definition of natural) will allow other RS members to understand the look you are achieving and knowing what implants you have (model/profile) will be helpful as well. Many women would "kill" to have the look of your left breast - the very augmented, round look. Do your homework by seeking a second and even a third opinion. Revisions can be complex, especially when downsizing. You need to select your PS very, very carefully and see many, many before and after pics of women who had revisions who are young and have a similar body frame as yours so that you can see whether or not you like the PS's outcomes. By all means, do NOT rush into another surgery. Take your time and do your homework so that you give yourself the best chance of getting the outcome that you desire.

I am so sorry that you have been disappointed. I went from a 36a to a 36b many years ago and I have been so happy. I agree with you - your bteadts don’t look ‘natural’. Gorgeous, but obviously your and your surgeon’s ideas of natural differ. I agree with Lori - get another couple of opinions. Don’t stick with your present ps unless you can now be certain he will do what YOU want. Good luck, and move forward carefully xx

I think if you change the projection and go under the muscle you may be happier. Depending on your BWD you could look very natural with a full profile or if you want to be very conservative you could go with moderate profile. I honestly think the profile of the implant was the wrong one the doctor chose especially from the wish pic you used. Sorry he didn’t seem to understand your goal. If you change profile make sure you pick the right size for your BWD so that it fits your chest and doesn’t look too narrow. Best of luck

I agree with bc88. Under the muscle will hide the implant more and lowering the profile will take away the "bubble look". Also look for a less cohesive gel. The firmness of the implant will also tell you how likely the implant to retain its round shape.
I currently am in the same position as you, high profile over the muscle, it's a nightmare but a fixable problem. Best of luck to you on your revision

Unfortunately communication was what caused this problem as profile wasn't even discussed prior surgery, I was basically shown some implant samples but they only variated on cc's, not profiles. I agreed to follow my surgeons suggestion 350-375cc as he said that's what I would need to get the look of my wish picture (see attached) he said it was impossible for her to have 250cc (that's what the model told me) and only now I found out that I got Ultra high profile! You can see the profile (which is what I hate the most because it makes me look super top heavy) on the second photo.
My implants were placed OVER the muscle because the surgeon said he decided that on the operating room as it depends on many things and he prefers to go over the muscle if possible in case in the future you need a reconstruction because of breast cancer... I found that a little bit odd but agreed to it. The thing is I did my research very well and this doctor is supposed to be one of the best in the country! He has all the awards and recognitions, his reputation is amazing and has been doing this since the 80's! It's a very stressful situation because I actually have to "rush it" because I'm leaving my country permanently in 2 months so I have to get it fixed asap. The doctor doesn't even recognize he made a mistake by choosing that profile... I feel so lost at the moment, I don't know what to do.

Hi. I am so sorry this happened to you. I personally could never go back to the original PS who does not have the requisite skill in order to do a primary breast augmentation under the muscle. I would start over and get 2-3 more consultations. You have already had these for several months and I urge you not to rush into something that might end up leaving you feeling worse than you already do.

Hi there, I just read all your updates to catch up. And I think your issues/questions may be better served in the doctors Q&A here. I also agree with the other member about getting a second opinion, but not because of your implant placement. I’m not an M.D. but it’s my opinion that your issue has more to do with the size/profile of your implants.
FYI there are a lot of biases (especially in the USA) regarding “overs” vs “unders”. In fact there’s a whole generation of doctors in the US that have only been trained to do orders; so keep this in mind when reading opinions on which is better. A PS that knows how to do both (properly) will tell you there are pros and cons to either placement... would select the placement based on your specs, goals and physical requirements. Btw, I have “overs” and most people that have seen my after pics think they look very natural. In fact some have told me they look more natural than many of the “unders” they’ve seen on these treatment boards. Good luck and please keep us posted! :-)

I was told that revising from over the muscle is an easier procedure for doctors because they create a new pocket and there's a high success rate with that. Moderate profile may be what you are looking for. I would go with a soft implant also as it naturally creates a teardrop shape when held upright and moves with the breast.

I have unfortunately had my breasts done 3x & never thought I would be the one to say that... I have tried it all! I had moderate saline 325 cc the 1st time with a fantastic result but after 24 years & zero problems, gravity had kicked in. So, I said let’s just pick them up a bit & I went to 425cc silicone - awful!!! More droopy than ever & smaller than the 1st time. Also one was smaller than the other because my saline had been filled to balance the look, but despite me telling my doctor this he didn’t weigh them & put 425cc in both size causing one to lol much smaller than the other (from nursing). 3 months later I was hating the saggy unbalanced breast so I went too extreme with the cohesive gel (aka gummy bear) 485cc & 525cc (saline cc for cc is not the same as silicone... no one told me that!!!) high profile. I am 5’10’” & they certainly were proportionate with my body but it was like Betty Boop! Within 1 year my breasts started to sag. I hated them! I was always a size 8-10 now I had to buy 12-16 just to fit the breasts & later everything, which usually didn’t look the best! Couldn’t buy anything off the rack! Nightmare!! My wedding dress, they made me order 16 to fit the breasts & the 1st tailor told me to return the dress 4 weeks before the wedding because it just couldn’t be altered as much as my perfect size 10 body needed!!! I was in tears but thankfully found someone to help with that.

It looks like one or both of your implants may have some capsular contracture (thick scar tissue that forms around the implants and makes the implants appear and feel firm and "round") and there is asymmetry - your right breast looks smaller than the left. Most women have asymmetry and whatever we start out with will be carried forward with a breast augmentation. It looks to me like you have some capsular contracture, but your PS will be able to confirm this.
If it were me, I would consult with a minimum of 3 plastic surgeons who are well known for revisions and make sure to look at their before and after pictures of women in the same age group as you and a similar body structure as yours. You are young, but your skin has been very stretched and you and your plastic surgeon will need to discuss whether or not you may require some form of lift with smaller implants. Again, I am very sorry that you did not get the results you were looking for, but with good communication, the use of "pictures" to describe what "natural" means to you and downsizing to a size that fits your dimensions, I think you would be very happy. The other thing that needs to be considered is the longer you carry too large implants, the more damage they do to breast tissue (drooping and thinning of breast tissue) as we age. With your careful research, I believe you will be able to achieve your goals. Your plastic surgeon will be able to tell you what size implants are appropriate for your dimensions in order to give you the "natural" look you are desiring. It might be possible that this results in a 100cc or larger downsize. For educational purposes, I think it would be helpful to read Dr. Pousti's comments on the Q&A section and to read the material on his website. I am not having my revision with Dr. Pousti (San Diego, CA) but he always gives great feedback on RS and offers very sound suggestions as to the importance of finding a qualified, experienced plastic surgeon, what to look for in a plastic surgeon and how to communicate your goals. I counted wrong and it's actually been 8 months, not 9 and I know I have to get the revision asap so the skin and tissue can recover better. Again, thank you so much for your answer. I'm going to go and reasearch Dr. Pousti's feedback :)
