I had a breast fat transfer with periareolar mastopexy and so far I’m very happy with my results. I’ve never liked my breasts and wanted to do something that did not involve implants. I came across Dr. Bucky when researching fat transfer.
Unfortunately we couldn’t do as large a volume of fat transfer as I wanted because I have extremely dense breast tissue with microcalcifications which requires me to get more frequent mammograms. A larger fat transfer would more fat necrosis more suspicious spots showing up on mammography which could lead to unnecessary biopsies in the future. So Dr. Bucky did a smaller volume and kept it to the upper pole to reduce this risk.
Procedure: fat was liposuctioned out of my saddlebags and transferred to my breasts. I also had a “donut lift” (periareolar mastopexy) which aside from lifting my breasts, made my areola smaller.
Post-op: upon waking from surgery, I did have significant breast pain, but I was given an oxycodone/acetaminophen 5/325mg (Percocet) which did the trick. For the first 48 hours I took my hydrocodone/acetaminophen 5/325mg (Norco) that had been prescribed which kept the pain in check. I didn’t need it past 48 hours. As for the lipo areas, it was somewhat painful, but felt more like soreness from an extreme leg day at the gym. Overall, the recovery was pretty easy and the pain was never unbearable. I was back to work in a week.
I’m loving the shape and perkiness of my breasts. Unfortunately, not all of the fat will survive, but I knew this going into it. At month 3, the fat necrosis should be done and I’ll have my final size. I’ve attached pics of before, 2-days post, and 4 weeks-post. The tape in the 4-week pic is silicone scar tape.

You look great! Keep us posted! Do you mind mentioning which type of lipo was used to harvest the fat? There are conflicting reports about different styles of lipo yielding different %s of viable fat for transfer, so I'm just curious -- either way, you're looking great!

Thank you! I’m actually not sure about the type of lipo though. Per Dr. Bucky’s website he uses the standard tumescent technique but I didn’t discuss the specifics with him.

You look amazing! Good luck with retention. Could I ask whether you wear a compression garment over your breasts? Have had mixed advice from doctors over whether to wear one

I’ve now hit the 3-month mark which means the fat that I’ve retained at this point should be permanent. I think I’ve retained a good amount. You can see in the photos that I still have a bit of “puckering” around my areola (from the internal stitches) but that will continue to improve with time. Overall I am happy with the results. My breasts aren’t “large”, but they are perkier with a smaller areola and a much nicer shape than before.

If I could have gone larger, I definitely would have! But since I already have microcalcifications and extremely dense breasts, we had to keep it conservative (because more fat transferred equals more fat necrosis which equals more suspicious spots on mammogram that may lead to unnecessary biopsy…since I already have issues with my mammograms, we didn’t want to make it worse).

I’ve added photos of the 2-month post-op point and the 3-month post-op point.

Thanks a lot for your pictures and review. Since we share the same shape and I'm interested in the very same procedure, I'll stay tuned and ask you if you might be interested in a 2nd fat transfer (after an undetermined time span). Take care and keep doing this great!

Thank you for your informative post. I have my consult with Bucky next week. Now that you are over one year out, have you still maintained same size? Do you still think it was worth the cost and pain?
