I am a 25 year old transgender female who has undergone HRT for over the last two years. I have wanted a BA ever since I started but never thought I would be able to have one since $$$ was always a factor. My employer (Denver hospital) not only performs BA for transgender woman but they also just recently started covering BA for trans employees (me) through our insurance company. My breast growth has been minimal and I am now around a 34A and I’m wanting to be a C cup.
Once I had the opportunity to get the BA (pre req: 2 years of hormone therapy), I asked my therapist and doctor to write me medical letters for the procedure.
After a few weeks the letters were ready and I was referred to a plastic surgeon Corrine Wong.
She consulted with me regarding what I wanted and all I could say was big breasts and for them not to be far apart from each other. I also really want cleavage.
I ended up liking the 330c range so she ordered 345 cc and 385 cc for an alternative. We also set a date for March 20th to do the surgery.
Before the consult I was so excited but after speaking with her regarding possible side effects I was all of a sudden hesitant to go through with the surgery. Even though I had educated myself of possible adverse effects beforehand, it had all become so real so fast.
I went ahead and scheduled the surgery date with tons of self doubt in mind.
Right now I am pretty 50/50. Going back and forth with my date still set. I’m posting a story in hopes of getting some inspiration and insight from others.

It is difficult to be sure from pictures, but my suspicion is that your current size is more likely to be considered a B cup than an A cup. As a trans woman myself, something that has been difficult for me to internalize is that the cup sizes on bras have to do with the size of the openings, not the volume or projection directly. You have wide breasts that do not have a lot of projection and which are angled a bit unusually. I suspect that because of your width, you need a wider opening than a 34A would give. I suspect that with a 34A that the bra cups are pushing against the side of your breasts with rub lines if you are using any kind of underwire. I know that myself, with my low projection, I have difficulty thinking of myself as anything more than 38B, but because of my breast width, I am considered 38D.

Your chest looks to be narrower than mine, which will make things a bit easier for you, but looking at your current breast separation and thinking about the research and testing I have done, then I would tend to doubt that 350-ish will be enough to give you "cleavage", at least in the sense of breasts that come within a finger-width of touching each other in a bra that is anything less than a fetish bra designed for the purpose of making them touch no matter how distorted that makes them.

When augmentation is done, the base of the implant cannot go much beyond the existing point where you flesh stops being stretchable away from your chest as part of the breast. You can get a touch wider than you have now, but mostly implants are fullness and projection. With that being the case, that the insides of the base of your breasts are not going to move all that much closer together, then in order to get cleavage (in the sense of the breasts nearly touching without being forced hard into that position) then you would need implants that were like mushrooms, narrow base expanding to wider -- and they would have to be fairly wide for the two sides together to nearly meet in the middle. Pancakes pretty much (and towards the outside of your body too, remember.) It isn't that it is impossible, but with only 350-ish augmentation, it is not at all likely.

I worked it out for myself a few times; I figured that in order for my own to get something cleavage-like in the manor my mental image wants, I would have to augment to roughly the G cup to H cup range... and that is starting with fairly wide BWD (Breast Width Diameter.)

I would suggest that you put a bunch more thought into what you want, and ask harder questions of your plastic surgeon about whether their proposed solution will give you what you want.

It is possible that your surgeon might say that you cannot get what you want in one step: that you need to do a stage now, let your tissue expand, and then do another stage a couple of years down the road. That is a feasible approach, especially so for transwomen. It is something that can be planned for. It is better to know ahead of time if that is how you are going to proceed because it can make a difference in choice of implants and incisions -- and of course it can make a huge difference as to whether you end up being really disappointed with your augmentation or if instead you are thinking of it as, "Good, good, I am making progress towards where I need to be!"

One approach I looked into a bit is that there are expandable implants, devices that can have fluid added through ports to either fine-tune the final size or else to permit you to stretch your skin further as you without risking those white expansion streaks that can happen if you go too quickly. I have seen differing opinions on this technique, with some saying "Yes, this is a good approach, but note that we would probably replace them later to 'finalize' them", and others saying "Well since you are probably going to do surgery twice anyhow, better to use the tried and true methods".

So, I suggest you look at more pictures and do experiments to quantize how big "big" is for your purposes. I know that if I push up with one hand and look from right side then my existing ones start to appear to be a reasonable size, but that otherwise I am often disappointed with the projection (only about 1 1/2 inches): talking about cup size doesn't tell you all that much about projection.

You should be sure to get clear with your surgeon what angle your chest is likely to mostly point afterwards. Your existing breasts point outwards kind of triangle like -- which is not at all uncommon for transwomen due to the curve of the bones. If your augmented breasts were to be (say) another 2 inches fuller but still point at the same angle, then are you going to be satisfied? Are you thinking of a firm implant (silicone) or a soft implant (saline)? That will make a difference as to how easy it is for a bra to move the flesh forwards and center...

I am a 60 yo. MTF trans. Had my BA six months ago and it is well worth the effort and expense. Good luck and best wishes. Please keep us up to date to how things are going.

I'm 28 yo, MTF myself. I had my BA in March of 2017. If you're not at your ideal weight, I would recommend going under the muscle. My surgeon said it would be fine either way and now that I'm losing weight (depression is an evil thing), I'm not happy with the feel or appearance anymore. Even more so now that I'm experiencing the onset of capsular contracture.

I'm still ecstatic that I went through with the process, but I would have done things differently had I known a bit more.

I was VERY nervous before the surgery, but I went through it anyway because I ultimately got exactly what I wanted. Larger breasts that helped me to feel more at peace with my body. I went from a 36A to a 36D and couldn't be happier with the size.
