I've been trolling RealSelf for over a year reading everyone's stories as I considered my own journey. I didn't think I would ever share my story, but since my situation is rather unique I am putting it out there in the hopes that it will help someone else.
I am in my mid-thirties, 5'6", 135lb and hovering around 20% body fat with constricted 32AA breasts.
I always knew my breasts were very small. Beyond that I knew that they were not the correct the shape. They always looked to me as if they didn't ever finish developing. I didn't know what it was and I didn't speak of it to anyone, but I always suspected that it had something to do with the fact that I was born with PCOS, a hormonal disorder. I've always been reluctant to pursue surgery because of the innate risks. But as I have become more health conscious and lost weight, they just looked worse and worse. I had to spend more and more effort trying to compensate with excessively padded bras, padded and formed swimsuit tops, and chicken cutlet inserts. It just finally got to be too much and I decided to at least explore surgery.

I totally get that!!! I'm glad once I made the decision that I moved quickly, it was a necessity for me because of my work schedule (small window of time to get it done) but boy you can go round and round questioning every decision and worrying about every possible complication. Deep breaths! Trust yourself and your decisions! That's what I tell myself!

With this condition its unreal to breastfeed a baby...ive tried but and it was a huge mistake

I haven't gotten to that point in my life yet but I have heard it's iffy at best. To be honest I anticipate not being able to breast feed (assuming I can even get pregnant due to PCOS)

My friend was diagnosed with PCOS after having 2 kids. She was told that she couldn't have children anymore. About 6 months later she was prego with TWINS! So you never know. Good luck.

Consultation and All the Things....

I only did one consultation for my surgery but the surgeon is indisputably the best in my area. I had a virtual consultation at first because of COVID. That was when I first learned that I did indeed have constricted breasts which, if I understand correctly, is the same condition as tuberous breasts but without getting the full tuberous shape (perhaps because they are also so small). I then did a follow-up in-person appointment.
Let me preface this by saying, I had no idea my boobs were so complicated. But I want to provide this information for anyone out there who might be in my boat. I hope it helps someone....
Possible complication 1) The surgeon explained that this surgery would be more complicated because of the constriction. There are bands on the inside that are pulling my tissue towards the center which is what gives them a conical shape. When I have my surgery, he will also try and release those bands "like an umbrella" as he put it, but some of the cone shape may persist.
Possible complication 2) He also said many times my type of breast also require a certain type of lift which pulls the nipple tissue back. Without it, there is a risk of getting a kind of "snowman" effect with the breast tissue topped off with an extra bubble from the nipple (like a snowman head on top of a body). He said I'm on the edge of needing it so we decided to just do the augmentation first and then if I am unhappy with the results we can do the lift later.
Possible complication 3) Lastly I also have risk of a "double bubble" on the bottom as well because there is so little tissue on the lower pole of my breast and it may not cover the implant. For that reason he advised me not to try and go too big. That wasn't a problem for me because I didn't really want to go bigger than a C.
All of that makes me very nervous!!! On the other hand, he showed me pictures of other women with the same condition and their results were beautiful. I know he is a very skilled surgeon so whatever I get, it will be the best possible.
OKAY so the fun stuff, tried on sizers. I want to be about a C cup, so he pulled out three sizes 300, 325, and 350 and I was between the 325 and the 350 but ended up going for the full 350! I'm surprised by myself because I have always said I didn't want anything too big but I feel like they just looked the most right on my body. I chose a silicone moderate high profile because I like the natural slope of the breast, and I honestly don't remember the style but I just said I wanted them to feel and look as real as possible.
In sum I am getting 350cc Natrelle Silicone Moderate High profile
AND I AM SCHEDULED FOR AUGUST 10!! That is ONE WEEK AWAY! Excuse me while I go run in circles screaming........
I hope I didn't choose to big. I keep thinking about it.....
Can't have anyone in the office with you because of COVID so here are some lousy self-taken pictures with the sizers on and also my wish pics that I provided (though, realistically it's probably too much to hope for. A girl can dream!)
Anyone have suggestions on what I need to get/do to prep?

First of all congratulations on your decision to have BA! Your surgeon sounds great! All those things he told you are absolutely correct in order to have a beautiful result. Def when ladies try to go too big with TBD it almost always ends badly. They end up with double bubble. If the PS doesn't release those bands, you'll have undesirable results. I've read in the Q&A forum that those bands need to be scored. It's awesome that he had lots of b&a photos of actual patients and their results. Def helps confirm your in good hands. *One thing I will tell you is to check on his revision policy since you mentioned about that lift if you're unhappy. Some people don't understand that most revisions cost money and are not free. Very few PS do free revisions.* Make sure you get all your post op supplies and I'd recommend to set up a small station next to you. I used a small shoebox and put straws, wet wipes, lip balm, a book/magazine(s) and meds. If you don't have any help with your medication put them in a weekly flip top pill dispenser or get easy to open bottles/caps. When going under engaging those muscles afterwards feels weird and really uncomfortable the first few days. I hope this has helped you out. Please continue to post updates. I'm super stoked for you!! Thank you SO much this is incredibly helpful and reassuring!!

Excellent advice about the lift. He did make it clear that it would be an additional expense. He said it would only be about $500 cheaper if we did it at the same time, so I'd rather risk the extra $500 than get a lift that I potentially don't need. Fingers crossed that it turns out well the first time.

Great idea about the shoe box!! I will definitely be doing that!

I see that you went up quite a few sizes, did you do anything to help prevent stretch marks? They look gorgeous!!

I will definitely keep posting updates, it's oddly therapeutic lol

I've been doing that thing that we do and going over and over whether I picked the right size or not.

I found this really handy app by Natrelle called "Natrelle 3D Breast Visualizer" in the apple app store that you can upload 3 pictures to + 1 measurement and they will simulate a 3D breast enhancement on your body at the size of your choosing (and you can compare various sizes too). I know it is an approximation, but it's nice to see the specific size on my specific body.

I'm definitely happy with the size I chose. I was worried about it being too big but the more I look at it, the more I feel it isn't too much. I think it will be really interesting to compare these photos to my after photos and see how closely they match.

I also bought some cleansing wipes, bendy straws, and an angled pillow for my recovery. It is getting so close now!
