Back and Flank Concerns After Surgery

Lower back and flanks update after 6 weeks
At six weeks post-op, I am becoming increasingly concerned about the area above the scar. As the tissues around the scar continue to heal, I can now grasp the tissue in question quite firmly, both horizontally and vertically. I will be interested to see how this area looks once the scar and surrounding tissues are fully healed.

Back and Flank Concerns After Surgery
Please note that my abdomen should not be considered when evaluating Dr. Ak’s work. I intentionally did not include photos of my abdomen because it was operated on approximately one year earlier by a different surgeon, and I am very happy with that result. I went to Dr. Ak specifically to address my back and flanks. My abdomen appears in this video only because the video was taken to clearly show the problem with my back and flanks.
I am currently still in the healing process, and I fully understand that the final result may take approximately six months to settle. I am not presenting this as my final result. However, I already have significant concerns about what I see.
Before the surgery, my main concern was the excess skin and tissue on my back and flanks. The procedure was described in my postoperative report as “Second-stage body contouring/reconstruction following massive weight loss – Back and trunk reconstruction with liposuction and excision of excess skin and subcutaneous tissue.”
The scar is now positioned substantially higher than the location I understood and agreed to before surgery. Most importantly, I can still pinch and hold a very large amount of skin and soft tissue directly above the scar, as clearly shown in this video. In my personal opinion, the amount and contour of this tissue looks very similar to what I had before surgery. This is my subjective observation, not a medical conclusion, but it is the main reason I am concerned about the result.
Regarding the liposuction, I am not claiming that it was not performed. I have had liposuction previously, so I have a personal point of comparison. After my previous liposuction, I experienced significant bruising, numbness and altered sensation in the treated areas. After this procedure, I personally did not experience comparable symptoms in the treated areas from the first day after surgery until now. This made me question the extent of the liposuction, but I understand that my personal observations cannot medically establish whether or how much liposuction was performed.
Because my concerns are significant, I have requested an independent second opinion from a plastic surgeon in the Netherlands, where I live. I have repeatedly requested my complete medical records from the clinic for more than three weeks so that the surgeon can properly assess what was actually performed. Unfortunately, I have not yet received the complete documentation, which makes obtaining a properly informed second opinion more difficult because the surgeon may have to work with incomplete medical information.
I understand that healing is still ongoing and that the final result may change. I am therefore documenting only my current experience and concerns at this stage. I sincerely hope that the result will improve during the remaining healing period, but at this point I remain concerned about the position of the scar, the contour of my back and flanks, and the significant amount of tissue that I can still pinch above the scar.
