Atlanta's Hidden Gem! LSR Back Lift + Tummy Tuck Combo

dr. castillo puts people before plastics. If you want that boutique like, personal feel, where everyone remembers your name & face go to him! If you want different than the overadvertised packed plastic surgery sweatshops promoted on ig this is the place. he's thorough & caring but no fluff. never saw me as dramatic or doing too much. he's just as particular about every little detail as me. seriously what I needed the most after my lifelong battle with weightloss. his work was the cherry on top.

Research:
I lost 120lbs naturally before scrounging the internet and going on plastic surgery consults. i have wanted plastic surgery since I was an insecure Middle Schooler watching Dr. Miami on snapchat, so this was the big reward to myself, that I have been saving up for, little by little! So I wanted to make sure I got the best bang for my buck. After 6 months or reading every realself review of every surgeon in ATL... I wasn't accepting anything other than 5 stars! I saw 3 surgeons in person (started with the free consults which were subpar, at best & I learned quick, you get what you pay for...) Dr. Castillo was my last visit. I'm telling you right now if you are debating on paying that consult fee - do it! I am a big believer in "when you know you know" and the peace I felt even on the phone booking a time with Angelica and on the way to the appt. was telling.
Consult:
Here are thoughts straight from my journal after leaving that consult Jan. 15th 2025:
leaving dr castillo office feeling: good, calm, in safe hands, i really want him as my surgeon! he's decisive. and opinionated but i love it lol
first- we just talked (clothes on lol) his only kinda concern abt my weightloss was the fact that i am still currently losing. and suggested i wait until im fully stable weight and happy with how most of my body looks. he was saying not to really focus on BMI too much because you may get to your BMI and be like I don't like the way I look so he rather I get to a comfortable weight. he was like some people get too skinny and feel like places the previously loved like their face now looks too thin I appreciate how weight loss was approached personally and delicately he actually sat down and had human to human conversation integrating his personal experience it didnt feel like a "dr. lecture"
THEN, the most life changing thing for me was us looking in the mirror together at my consult! Felt so less judgey then when a dr just asks you what you want and looks at you to see if they can do it or not. And as a massive weightloss patient their is loose skin just about everywhere lol so he had me name what i "hated" in the mirror most to least. in priority order. Then he gave his opinion on what could be done together and what would give me the most "bang for my buck"
he suggested bra backline lift, extended tummy tuck first (id also be a candidate for fleur de lis, but im not a fan of that scar and he said he doesn't really recommend it on younger patients bc im still wearing crop tops etc. dating - he also made a good point that you can always come back later and get more taken off but you cant ever go back when you make that decision, This talk through really was a blessinggg for my indecisive brain!
He showed me more recent before and after pics on his in office Ipad - i love how he does his new belly buttons more slit like. Alot different than what I saw on his real self profile. love how he had me bend down in the mirror too and showed me how far the scar would have to go for the extended tummy tuck. He was real about the risk of dog ears and my personal risk of belly button necrosis but explained how he fixes these things. I appreciate how real he was about how lipo alone (on my back) wont do much for me, & how it will make loose skin hang more lol.
then we talked through other combo options like mommy makeovers, arm lift, thigh lift.
he reaffirmed that the fact that I have already done a great job and non of whats left couldve been lost in the gym.
also surprisingly he suggested that i may want a lower body lift later where they extend my tummy tuck 360. down into my butt crack and it would give me the butt pop i kinda am looking for when training glutes.
It didnt feel like an upsell though. It felt like a genuine suggestion and his positive attitude remained when I politely declined. I have never really cared abt that area before but i didn't take offense to the suggestion
Most of all! I appreciated how he took his hands and lifted each area (back/arm/hips) to show me how it would look & the benefit from each specific lift.
Back had such a crazy difference that i saw it right away in the mirror. and was like wow. I was never a fan of the braline scar because of backless dress, and halter top dresses. Dr. Castillo is so funny and real and straight up, he asked well do you wear those type of dresses now? ....lmaoao point taken. So it made me think, how big a deal is a scar really? He was straight up that my back had the MOST loose skin, and thats what he would suggest doing first. Really like how everything felt personalized. He didn't just say what most people did. He said what will work best for me.
What set him apart is also, the fact that he will do a TT and Backlift in the same surgery. A rarity when most dr's like to stick to the same side and stop at the limit of mommy makeovers. This made this consult feel so personal to me, like we were coming up with a game plan specific for me, not some cookie cutter formula.
He kept it real with me. he said "you are never going to be drum tight. Don't expect it - there will always be some remanats of your previous weight/weight loss" this was a hard truth for me to hear but one i can accept it now. It shows me how strong i am and how proud i should be that i changed my life around.
Angelica, the supervisor scheduler also gave me great info... sent me over multiple quotes for the different combo surgery options as I was still deciding! As someone with no previous credit cards she explained it to me all the different types of loan programs in plain terms which i appreciated. I also like how both Angelica & Dr. Nelson Castillo recommended that after you watch all of those videos and you book your surgery to stop watching them because all they do is put fear in your head like they're really good informative at first and then when you watch too many it's like you're seeing the same thing over so you're not learning anything new but you're creating more phobias in your head. and I thought that was really amazing how they cared about the whole patient: mind + body because this is a big deal and it's a lot to cope with especially at 25!
Dr. Castillo continued to email me, check in, and answer any questions even before I booked with him! And I love that he responds QUICK AF - he will email you back at the crack of dawn, call you, or have you come in for a follow up consult at no extra charge if you need to see more pictures or need more clarity on something.
Being so young, most dont understand why I would do this now before I had kids (my own mom was against it :/ ) but I really valued my research in finding the safest way possible, with a highly rated dr that graduated from reputable well known schools like emory and harvard. And my surgery being done at a top hospital I am familiar with like Northside really helped reassure me, that I would be just fine in great hands from the pre and postop nurses to the anesthesiologists to the surgery techs!
Booked in October 2025:
Yes 10 months after lol. Angelica was great about calling me and keeping in touch (but they never made me feel rushed) so I could make sure I booked before hospital facility prices went up for that next year! Even though next year's surgical calendar wasn't in place, she had the date I wanted noted. and kept calling Northside to make sure I was first in line to get that date as soon as the May calendar opened. Angelica, put up with my phone tag and was always being so sweet and helpful with keeping up with my balances. Loved her just as much as the Dr. She gave her own personal experience when needed and she was such an amazing listening ear. Her and Dr. Castillo are a dynamic duo. They were always able to fit me in their schedule and put up with my Type A ness and for that I'm very appreciative.
Pre-Op:
Everyone is so Joyous in the office! They are counting down the days and excited with you! I kind of inquired about like pictures or diastasis measurements and he is no bs okay, lol he let me know he is their to work not be a photographer. and he definitely doesn't have time to measure stuff that doesnt really matter! he's in their to cut and at first I was a lil sad but I appreciate him valuing my hard earned money, by focusing on the task at hand. Because every minute in that OR cost about a hundred dollars lol and he does not like to waste it!
The Day Before Surgery :
Dr. Castillo had me come in to get marked so he could take his time and make every line perfect, he laid me down as if I was on a surgery table, and its that attention to detail that I needed! I'm telling you he measured 10x in preparation to cut once. And he took pictures just to make sure it was perfect on camera not just to the naked eye. and to show me! He was honest about the middle ground needed to pull me tight but also not too tight to where I had weird ripples or dog ears. He talked about his preference to make his tummy tuck lines the perfect classic natural bikini curve that will stand the test of time Versus the straight across or super high thigh scars. But still asked If I wanted something different, I chose to trust him! He is so personable while he marks and though it was a long process it didnt feel awkward at all. He was real about what to tell my family, who would be waiting on me for a long 7.5 hr surgery (they do not need to wait all day!) Ren at the front desk was so encouraging and calming to my pre-surgery jitters and didnt mind that I called right after my appointment when I forgot something they told me!
Day Of:
He was the first person I saw when I was called back from the waiting room and that made me feel so at peace.
He touched up all of his marks, and explained how alot of different people would be coming in, OR cold warnings, etc. but he was clear on when he would see me again, what he was going to set up, etc.
And he was the last person I saw before going to sleep so I felt like I could really trust him.
I appreciated how he didn't overwhelm me personally with information after the surgery just updated my family members.
To no fault of his own, Their was an error in the surgical materials count during close (nothing was found thankfully) but he made sure Northside billing was notified that they would cover the cost so I wasn't charged the xray or anesthesia overtime fees. And this wasn't brought to my attention until a week after - very glad because it might have scared me if i heard day of lol.
Post Op:
He called that night to check in on me and every day after for 2 weeks! His job doesn't stop on the weekend he will answer an email or call no matter how small. And when you freak out he will remind you of the truths of where we are right now. The pain for me was NO JOKE hurt really bad despite the meds, (take in mind i have never had kids and this is my first real surgery or pain in general lol) But he worked to get me something stronger ASAP. When I was itchy he had us go full fledge into all of the allergy medicines to get ahead of it and help my hives.
He took drains out as soon as they were ready, even on an early saturday morning.
Every small victory was celebrated by him, he found jokes to help me get through the pain and was honest about when I would have to just bear it because it is temporary.
I really appreciate him, I've been wanting this since a little girl seriously and cannot recommend him enough - I never thought I could be this beautiful! I've been square shaped my whole life but somehow he found curves for me under all this loose skin!
And the fact that it will only get better from here as swelling goes down blowss my mind!!
