I have been using fillers for fine lines for over thirty years with gratifying results, dating back to the original days of injecting Zyderm collagen in the early eighties. Static fine lines, including those in the delicate area of the crow's feet around the sides of the eyes can ideally be plumped up and straightened with the judicious use of fillers. These days we are fortunate to have fillers like Belotero Balance, that, when injected very superficially as would be needed in these cases, has little chance of causing the unwanted, bluish Tyndall effect seen when other fillers are injected superficially. To further prolong the effect, a small amount of neuromodulator, such as Botox, Xeomin or Dysport can be used to in the same treatment area to diminish any dynamic contribution that the small muscles around the eyes have in forming the static lines. The combined use of filler and neuromodulator has also been shown to prolong considerably the duration of effect.As to overally facial rejuvenation for evening out blotchy pigmentation, roughness, hyperpigmented spots and crepeyness, I have found that a series of superficial peels with high concentration glycolic acid, Jessner's solution of low strength TCA to be particularly effective, especially when combined with a home regimen that includes topical retinoids and alphahydroxy acids. In general, I have been disappointed with the results of most lasers, including fraxels, and do not feel they warrant the time and expense they entail. Consultation with a board certificied aesthetic core physician with experience with all modalities is a must.
