There are several different texture surfaces to implants and the data is strongest with regard to the association of ALCL specifically with Natrelle (formerly known as Allergan and McGhan) textured implants. Although the use of textured implants have largely fallen out of favor, there is little concern over the Mentor Siltex textured implants. If you are happy with your result and not having any problems, you do not have to have your recently inserted implants removed. Discuss your concerns with your plastic surgeon, but other than following the normal guidelines for breast monitoring you should enjoy your result and relax. Good luck.
BIA ALCL is exceedingly rare.The recall does not include asymptomatic patients. Many manufacturers have pulled their textured devices out of an abundance of caution.For those symptomatic patients (asymmetric swelling with seroma), recommendation is for fluid sampling and subsequent explant with capsulectomy if positive.With regards to your specific question, if you are asymptomatic nothing needs to be done. The rarity of BIA ALCL does not warrant across the board pre-emptive removal.As always, discuss your concerns with a board certified plastic surgeon (ABPS).
It is recommended that if you have a textured implant in place, due to its association with ALCL, you should have them removed or exchanged to a smooth implant.
All implants, textured and smooth, do have some risks, but the risk is much much lower in smooth implants. The biggest risk was with Allergan texturing because of the process used for the textured. Mentor textured implants are imprinted and are smoother than the Allergan implants. If you are doing well, just leave them alone. However, I do recommend getting an ultrasound or mammogram every 2 years to be checked. ALCL is still a rare tumor and you have a higher chance of having breast cancer from the other types because you are female.
