Does this spot require dental work? (Photo)

I was wondering if this was a cavity or exposed dentin, been there for three 3 years.

Without X-rays and an Exam, I cannot provide an definitive answer. But this image looks like damaged or thinned enamel. While it does NOT look like a cavity, I DO recommend that the defective enamel is removed/replaced with a filling. Defective enamel is more porous, and can develop cavities over time. 
It is also possible this is fluorosis, but since it is affecting only one tooth its less likely. I would still recommend this be filled, so that your smile is more esthetic if this is the case, but it is not necessary.

You can see that the cavity has progressed, it usually starts with white patches, then turns into a yellow patch, and finally becomes black in color. So, you can consider this as the second (moderate) stage of the cavity because I can see a white patch around it, and the middle portion is yellow.
It is advisable to visit a dentist and get it treated because, otherwise, if it goes deeper and affects the pulp, then a root canal will be the only option.
If you go for a cavity filling at this stage, its life is very long. But once a root canal is required, the life of the tooth is around 10 years.
