Doxycycline to spironolactone: My skin is great but my derm want to change my prescription, what should I do?

Thanks for your question.  I can completely understand your hesitation in stopping the doxycycline, but I agree with your dermatologist - you don't want to be on antibiotics for too long, and it probably is not really working anymore at this point.  I would encourage you to talk to your doctor about transitioning to spironolactone and gradually weaning off the doxycycline.  If you aren't using any topical medications, you might want to start a retinoid and possibly a topical antibiotic or a salicylic acid or glycolic acid wash, based on your dermatologist's recommendations.  The topical retinoids can be really useful for maintenance after clearing up the active and cystic acne.  Best of luck!

Side effects of long term use of oral antibiotics include antibiotic resistance, which is likely one of the reasons why your provider is switching oral medications. In addition, do you use topical medications for your acne? Acne treatment may involve multiple medications, including oral and topical, as well as aesthetic procedures such as facials.
