Best way to prevent relapse after premolar extractions?

My teeth seem to relapse pretty fast even in retainers, perhaps because the clear retainers have give and the Hawleys don't cover all surfaces of my teeth. I had premolar extractions + widening (not palatal, just teeth). Is there a combination of retainers that are best for prevent a narrowing & inward collapse of my teeth? I'm noticing my smile keeps narrowing and my upper incisors keep extruding. Also, could bone loss from my premolar extractions be continuing 3 years after extraction?
