Emsculpt Neo - does apoptosis only occur where the paddle touches?

This is a wonderful question! Apoptosis (for those who may not know) is cell death. The radiofrequency with Emsculpt Neo precisely heats the tissue to 42 C, which causes fat cells to swell and burst. The dead cells are then carried away by your lymphatic system.
When the paddle is applied, the heat does spread out along the subcutaneous fat layer, so apoptosis occurs under the paddle, but also spreads out beyond the area of the paddle. There are several clinical studies showing cell death using many factors to measure it; I'm happy to provide those if needed.
 

The fact that the company has proven but a few fat cells broke open during some form of treatment using this device does not make it an effective fat removal device.

This device primarily gives temporary muscle strengthening.

If you’re looking for this to remove substantial amounts of subcutaneous fat to the point of seeing a visual difference you’ll be disappointed.

The gold standard and only procedure that will do that effectively on a consistent basis is Liposuction.
