Should I opt for an implant if apicoectomy with bioceramic root sealer failed? (Photo)

It’s 8 days since apico on 21. Looks like it didnt work coz I have the same severe trobbing pulsating pain as before the operation. I got 2nd pack of antibiotics even though on x-ray it “looks good”, told me to wait 1 more week and if pain is still there, he wants to pull out the tooth. I went for 2nd opinion, and they said based on CT scan it looks like there is still lesion at the back of the root. Can I redo the apicoectomy now? X-ray before apico attached for idea, i dont have one after.

You should feel better with apico after one week.  If within two to three weeks its not better, then most likely extraction and bone graft is in order.  In four months you should be able to receive the implant in that area.  The X-ray that you have uploaded is of the upper front tooth and not #21.

Hi there! Sorry for what you're going through. Wait another week or 2 to see if your symptoms resolve.  If not, then it may be best to have the tooth extracted and have an implant placed.  Implants are great restorations and are a great way to replace missing teeth!

wait for 7 to 10 more days. pain is still there means , infected root. in that case, remove and plan implant .

I am sorry you have been going through such a difficult time in regards to # 21. I would recommend opting for a implant if a second attempt at an apicoectomy is necessary. Implants are the standard of care. Please make sure you see a oral surgeon or periodontist to evaluate and place the implant. Good luck.
