I am a 23-year-old male, 53 kg, 6 ft tall. I’ve been losing hair for four years, starting at 19–20 during a period of poor diet, high stress, and bad sleep. Seborrheic dermatitis also began around the same time. Initially, I thought it was stress/dandruff-related, so I didn’t treat it, but hairfall persisted. Multiple dermatologists diagnosed AGA and advised finasteride and minoxidil, but I didn’t believe them because my hair loss is diffuse, my hairline is intact, and I didn’t want lifelong medication. I tried home remedies and PRP, with no improvement. Four months ago, I started minoxidil 5% and finasteride 1 mg; hairfall slightly reduced, but there was no regrowth. Finasteride caused libido issues, and minoxidil worsened seborrheic dermatitis, so I stopped both. It’s been a month since stopping; scalp is still irritated. All deficiency tests were normal except vitamin D (16.9 ng/mL). I stil lsuspect this may not be AGA because hair loss is diffuse, hairline intact, family history is negative, onset coincided with poor lifestyle, and seborrheic dermatitis may contribute.

Diffuse hair loss in a 23-year-old male can absolutely raise the question of whether this is androgenetic alopecia (AGA) or another process such as chronic telogen effluvium (CTE) — but diffuse thinning does not exclude AGA.

Early AGA in men often presents with diffuse thinning over the mid-scalp and crown while the hairline remains intact, especially in younger patients. Family history can be absent, as AGA is polygenic and may skip generations. The chronic, progressive course over four years is also more typical of AGA than telogen effluvium, which usually stabilizes within 6–12 months after a trigger is corrected.

That said, your history of severe stress, poor nutrition, sleep disturbance, and vitamin D deficiency (16.9 ng/mL) could certainly have triggered a telogen effluvium initially. In some patients, TE can unmask underlying AGA, making the thinning appear more sudden or diffuse. Seborrheic dermatitis does not directly cause permanent hair loss, but chronic inflammation can increase shedding and worsen scalp symptoms.

The lack of visible regrowth after four months of treatment does not rule out AGA — that timeframe is typically too short to assess meaningful regrowth. Libido side effects with finasteride are a valid reason to reconsider systemic therapy, and scalp irritation from minoxidil (especially if alcohol-based) is common in patients with seborrheic dermatitis.

If there is still diagnostic uncertainty, a dermatologist can perform dermoscopy or a scalp biopsy to distinguish AGA from chronic TE. Based on your timeline and pattern, AGA (possibly diffuse pattern) remains the most likely diagnosis, potentially compounded by past lifestyle stressors.

This is quite likely AGA for at least one of the diagnoses. I agree that the question comes down to. Is there another diagnosis present as well other than seb derm ??? The only way to really diagnose hair loss is to have a full history and to examine the scalp of close, including trichoscopy.  There really is no other substitute. A good clinician needs to see your scalp up close in this case. True CTE or so-called chronic idiopathic telogen effluvium does not typically occur in your age group so that’s very unlikely.

Your focus on the intact hairline is incorrect as many males do have an intact hairline especially when AGA is early onset or with certain racial backgrounds including Asian. Your emphasis on this as an indicator this is not AGA is flawed and incorrect.

All in all, I would encourage you to see a hair loss expert and have a biopsy if you or your dermatologist is uncertain with the diagnosis. We don’t need a diagnosis in all cases, but if the diagnosis is uncertain after a proper history and proper examination and it can be helpful addition to the diagnostic work up. It is quite likely that you have androgenetic alopecia as one of your diagnoses and I would not discount discount that too quickly.  Be sure to get expert advice. Nothing beats that.
