5 months post. Wound open after chin implant. Do I need to worry or fix something?

The risk of infection following the placement of a chin implant diminishes dramatically months after the surgery. Since you had an intra-oral approach, it appears that there is some webbing of the vestibular mucosa.... There is not a risk for teeth problems or further infection, but I would follow-up with your surgeon and ask that the webbing be removed. It is more of an oral hygiene problem whereby food can be trapped in the vestibule. Good luck 

It is a very unpleasant complication of chin implant surgery. That wound may heal without any consequences regardless that you may have exposed bone/teeth. However, if there is a contact of implant with oral cavity (mouth) it may result in implant infection and finally removal. Antibiotics, frequent visits to your surgeon for follow up as well s excellent oral hygiene is recommended. Good luck.

This is not good for your tooth. It will be okay for awhile but it is not good to have root shown for a long time as it can be damaged easily. Please check with your surgeon.

I personally do not use an intra-oral incision for chin implants. I prefer a small sub-mental incision that has limited issues with infection, or possible nerve damage and movement of the implant.  Hi, I have performed many facial shaping procedures, including Chin Augmentation with dermal fillers or silastic chin implants, for over 30 years.  This isn't a normal occurrence and you should consider going to your chin implant surgeon for an evaluation.  Prolonged healing and possible wound dehiscence are just a few of the possible side effects of using an intra-oral incision for chin implant placement.  While placing silastic cheek implants through the mouth rarely has issues, the placement for chin implants seems to present much more of an issue which is why I prefer using a small, curved incision under the chin. When the chin is weak, this creates an imbalance making the nose appear larger, the mid face top heavy, the lower face looks short, de-emphasizes the lips and allows early formation of a "double chin".  
 Proper placement of a silastic chin implant adds forward projection to the chin thereby creating harmony and balance to the lower face.  Using the same incision, liposuction can be performed to reduce the fat and further shape the neck.  Excess skin, from below the chin, can also be removed through the same incision.  
 I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty (requires extensive tissue dissection, bone cuts and placement of metal screws and plates to secure the cut segments of bone).  I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone.
