I spent 5,000USD on a design/manufacture of my custom jaw implants which where were then sent out to my surgeon. However I am now considering going to a different surgeon but I really like the original design and want to use that same implant. Can I transfer that manufactured implant from my old surgeon to the new one I plan on going to? Has anyone had a similar experience?

The wrong implants can indeed make you look "fat." One of the premises of custom-designed implants is to make sure you don't look "fat" or out of harmony.
You have a somewhat round face, and designing the right implants for you could be challenging. If you have high jaw angles and your surgeon...
The screw you see as "hanging loose" may be, in fact, inside the implant itself; it does look like that one screw missed the bone, though.
Omnipor or Medpore is radiolucent for conventional radiologic studies. With a bit of work, you could see it with various software.
