How important is dental midline? (photo)

I had a very bad orthodontic treatment with asymmetrical extraction from lower left side of the jaw (premolor and canine). Now I am planning to get premolar substitution to get my bite back. Dental midline of lower jaw is shifted towards left side (about 5mm). So I want to know how important is dental midline. Does it cause functional issue or its all about aesthetic.

upper midline upto 3mm; and lower midline never matters. if yr bite is proper thn no need to worry, its my advice to upload photos , so i can guide u better.

Lower midline would not have any functional deficit in your overall treatment.  As long as your bite is adequate, the lower midline discrepancy would not have any long term effects.

Bottom midline is not as crucial and critical as the top midline for Esthetics. Functionally, midline being on or off is not that big of a deal as long as your bite is balanced and front teeth are touching. You’ll be fine.
