How important is dental midline? (photo)

I had a very bad orthodontic treatment with asymmetrical extraction from lower left side of the jaw (premolor and canine). Now I am planning to get premolar substitution to get my bite back. Dental midline of lower jaw is shifted towards left side (about 5mm). So I want to know how important is dental midline. Does it cause functional issue or its all about aesthetic.

If you continue to smoke, you will most likely lose your implants, and I guess that would be a problem for you. I have never seen any reports regarding negative effects of nicotine gum; so that should be safe. Absolutely stop smoking.
