I am 43 years old and have an overgrown maxila and very narrow arch in my jaw. I went to a reputable local orthodontist, and she said this could be marginally corrected via TADs and three years of braces.. marginally. She recommended I just get orthagnathic surgery. Since I am "old" and have a career and young children to manage, I didn't want to go that route. However I would like to get additional opinions on what can be done to fix my smile.

By seeing your photos, I feel you have dental class one but skeletal class three. So it's my advice to visit the orthodontist. Cephalometric analysis is a must in such cases to understand the perfect scenario. I feel you have skeletal excess.
Assuming your teeth are cavity free, gums are healthy, and you have appropriate root lengths…
Ideal treatment would involve:1. Clear aligners to level and align all teeth, expand the arch form to fill in some of the negative space on the sides, and create equal spacing around the peg...
You would want to see an orthodontist first to level and align the teeth. The malocclusion can be best masked through braces rather than a restorative solution.
