I have found the same question, but the answer wasn't definitive enough. I'm worried that if I smoke even an hour after I use the gel (it says to do so for 20 minutes) my teeth will instantly go completely yellow again, or be even worse because of the enamel sensitivity. I would like to know if it gradually reverses the effects of the gel (like it gradually strained the teeth in the first place) or instantly? If instant, how long should I wait for this sensitivity to go away?

We recommend you to make crowns on your teeth both in color selection and shape selection. You don't need to have your teeth removed. It is possible to close the gaps with crowns. The name of this ailment is fluorosis and your teeth become weak. Getting crown also strengthens your teeth.
I would suggest getting Sensodyne toothpaste and use morning and night to help relieve sensitivity! You can also take the toothpaste and wipe it across your teeth (without rinsing/brushing it off) for a couple minutes. Also, stay away from cold foods for a few days!
It would not affect the tooth, the other tooth areas will look whiter, we have to do a Restoration that matches the color achieve by the whitening procedure.
