Minoxidil on Donor Area Before Hair Transplant? (History of Alopecia Areata) (Photos)
Hello doctors, I’m a 35‑year‑old male scheduled for a hair transplant in about two months (frontal hairline). I’ve been using minoxidil on the top/crown for almost 10 years. My donor area is overall strong and dense. However, in the past 4 years I had intermittent alopecia areata episodes in the lower occipital region. Each episode resolved, but the patches recurred occasionally in nearby spots. Currently the AA is mostly inactive, with only a 2 very small spot remaining low on the back of the scalp. The rest of the donor looks healthy and very dense, though I feel that some follicles in the previously affected zones may not have fully regained their original density. My question is: Would it be advisable to apply minoxidil to the donor area now (and stop it 2–3 weeks before surgery as recommended), to possibly stimulate recovering follicles? Or is it safer to avoid minoxidil on the donor entirely before surgery to prevent any temporary, misleading cosmetic effect- which might cause weaker AA‑affected follicles to appear stronger than they really are and risk harvesting grafts that are not fully stable? Additional Question: Is it recommended to apply a topical steroid such as Dermovate (clobetasol) to the small active alopecia areata patch before the transplant, in order to accelerate remission and ensure there is no active AA in the donor area at the time of surgery?
