Threads should never be left poking out of the skin. From your description, it is likely that the end of the thread got caught when passing through the skin. With the facial movements, it can start protruding. Your doctor will be able to snip off the end of the thread, so you must let him/her know about it and go back to the clinic to correct this issue.
If the thread was not poking out at the time of the procedure, it must have moved during facial movements or manipulation are your part. The provider should see it and determine the best next step.
