Are fillers a good option for dent/hollow to side of eye? (Photos)

Hi all I have had tear troughs and cheek fillers in the past. I sleep badly and my eye bags/ hollows were very bad which it has helped with. However I think the cheek filler in the past/ combined with still sleeping badly etc has left me with these strange dents at top of cheekbone on the fside of my eye socket. There is literally almost a small hole of a dent. Not just standard eye bags. Is there anything that can be done- ie. Filler placed specifically to target this Please? It is really really depressing me.

Although there is no substitute for an in-person examination that allows for not only close-up examination, but for direct palpation and manipulation of the skin, it appears from the photos to be a rather common problem--one, as a matter of fact I have had myself-that can be quite easily treated, as I did for myself, by superficially injecting a small amount of a low viscosity, malleable, cohesive HA filler. The injection must be superficial to simply elevate and smooth the dent. You would be wise to consult with an experienced, board certified cosmetic dermatologist filler injector expert and best of luck.

As a dermatologist, I see this concern quite often, and I understand how frustrating it can feel.

From your description, this sounds like a lateral under-eye hollow (palpebromalar groove) rather than a typical tear trough. This area can sometimes look more pronounced after prior filler, especially if volume was added more centrally or if filler has shifted slightly over time.

The good news is that this can often be improved—but the approach needs to be careful and individualized.

In many cases like yours, I first consider whether existing filler is contributing to the contour issue. If so, dissolving a small amount with hyaluronidase can help reset the area. After that, if there is a true hollow, very precise placement of a small amount of filler deeper along the outer orbital rim/upper cheek can smooth the transition. This has to be done conservatively, as the skin in this area is thin and unforgiving.

It’s also important to evaluate the entire midface, not just the dent itself, since cheek structure and support play a big role in how this area looks.

Overall, yes—this is treatable, but it’s not simply a matter of “adding more filler.” A nuanced approach will give the most natural result.
