When I smile my chin protrudes down - advice on treatments or surgeries to fix this? (photos)

When I smile my chin protrudes down (side profile pic) and I get lines/folds on my face. I've been told that this is a muscle thing & I need to be careful using filler & that there's not much that I can do. Any advice on treatments or surgeries much appreciated! In addition the excess skin/fat under my chin. Thank you in advance!! :)

​I find that VASER liposuction in combination with Renuvion or ThermiRF are best for contouring the chin in those who are candidates, but Kybella can be used as an alternative and gets better results when combined with Thermage/Ultherapy/Exilis, TruSculpt iD, or Coolsculpting. A close-up, in-person examination would allow for a more thorough evaluation. From the photos, however, it would appear that a properly performed HA filler chin augmentation, using a robust HA filler, would not only augment forward chin projection (and hence diminish the the downturning seen), but, with likely secondarily help to better define and straighten the jawline and even help to lift some of the submental sagging. Make sure to consult with an experienced, board certified cosmetic dermatologist and be sure to ask to see his/her personal before and after photos before agreeing to proceed. Best of luck.
