On July 31st I had an implant placed in my upper jaw. Three weeks later it came out. I went back to the oral surgeon and he put in a bone graft. After 2 weeks the bone graft fell out. What is making this process so difficult? I am not chewing on that side so I don't know why it would keep failing.

You absolutely should have this examined by the surgeon who placed the graft. The graft well might need to be done over and you should be on antibiotics.
