If you choose to extract premolar teeth you will have to over retract your upper front teeth in order to close the excess space created by the extractions. When you retract front teeth the face will be retracted with the teeth. Imagine your face below your nose being moved back but your nose staying where it is. Your nose will appear larger, your upper lip will appear smaller and your face and profile will definitely change. Your smile will also be more narrow. Instead of extracting teeth to improve your overjet, consider developing your upper arch and moving your lower jaw forward. This treatment will improve your profile, maintain your lip support, maintain a broad smile, and improve the health of your airway and TMJ. Surgery is NOT required to develop your upper arch and move your lower jaw forward. Some dentists will tell you this is not possible at your age without surgery but there are many thousands of dentists who do this every day with simple, comfortable, removable appliances.Good luck!
