Struggling with skin and laser recommendations for sensitive skin - uneven skin tone, pigmentation. What would be best? (Photo)

Hi! I'm struggling with making a decision/knowing what the best procedure for my skin would be. I've gone to a few different dermatologists specifically with the goal of fixing my uneven skin tone (I have some clogged pore/pimples right now too) and pigmentation on, but not limited to, my forehead and nose. I also have super reactive and sensitive skin, so tretinoin (even niacinimide) was not compatible with my skin. I also think I have a super compromised barrier (so many products cause redness/burning, even sunscreen), so don't want to do something that will cause more issues. One dermo recommended treating the brown spots with laser (I think Pico), another with Fraxel. I've read that Fraxel is heat/might not be a great fit, Pico is super targeted (I'd prefer an overall treatment), and even read about IPL. ; genuinely am at a loss for what the best treatment or action is. Any advice please?

Thank you for sharing the details of your skin history. From what you describe, I would be cautious about choosing a laser first while your skin is still highly reactive.
When the skin is this reactive, even otherwise appropriate treatments can cause more irritation, redness, or post-inflammatory pigmentation. I would first focus on identifying what is driving the sensitivity and calming the skin before deciding between Pico, Fraxel, IPL, or another option.
Once the skin is more stable, treatment can be selected based on the type and depth of pigmentation, skin tone, texture concerns, and how reactive your skin remains. Picosecond lasers can be useful for certain pigment and texture concerns, while fractional lasers such as Fraxel may be better suited to other indications but can involve more inflammation and downtime. IPL can also be helpful in selected patients, but it is not the right choice for every type of pigmentation.
In a case like yours, I would prioritize a careful skin assessment and barrier recovery first, then introduce treatment gradually rather than trying to correct everything at once.
