I have verry narrow upper arch (as seen on photo), narrow smile that is just getting more narrower by year, I also have difficulty breathing and posture problems. I did alot of research lately and saw that there was successful stories of palatal expander for adults but I also saw people talking about DNA appliance. My orthodontist told me surgery in my situation would be best, and also invisalign or braces to slightly tilt my teeth but that cannot expand my arch. I would really want a second opinion

It sounds as if you are on some sort of insurance plan that dictates who gets braces. If you are paying cash for your braces there is no acceptance or denial process. I would guess your insurance is the problem.
Not wearing your retainers has allowed your teeth to shift. Wearing the retainers again may move them back into the ideal position or it may not. I suggest visiting your orthodontist to determine the best course of action.
Lips are guided by teeth positioning. If there is an overbite, there may be protruding lips. Evaluation by your dentist should tell you if this is the issue and how to correct it. Orthodontics is the typical treatment recommended.
