Much of my dental practice is involved in undoing the ill effects of bicuspid extraction orthodontics. YES, when you extract bicuspids before doing braces you will OFTEN adversely change the appearance of the patient's face. Taking bicuspids out to relieve dental crowding will make the patient's face longer and more narrow. It will make lips smaller and a smile more narrow. It will often trap the lower jaw in a retruded position thus adversely affecting the TMJ and patient's airway. In short, extracting bicuspids to relieve dental crowding is RARELY the best course of treatment. I always recommend a patient get multiple opinions before ever agreeing to extract bicuspid teeth before braces. Today's modern orthodontic techniques give far better results from both an aesthetic as well as functional basis. Undoing the ill effects (aesthetic and medical) of bicuspid extraction orthodontic treatment is time consuming and expensive. It would be far better to use better techniques from the start rather than having to undo and redo.Good luck!
