Adult with overjet, deep bite, and facial asymmetry — expansion vs extraction vs surgery? (Photos)
I had Invisalign from Dec 2020 to 2023, which straightened my teeth but reduced my overjet by only about 1 mm. I was told it would not fully correct it. Another orthodontist had suggested wisdom tooth removal and a palate expander, but I did not pursue that at the time. Recently, after seeing photos of myself smiling and noticing that my front teeth still appear prominent, I sought a new consultation. Initially, my orthodontist said I could leave things as they are, but my bite feels off and uncomfortable. I was given a deprogrammer to wear for 14 days to determine my true bite. After that, the plan may be braces with a unilateral premolar extraction or braces combined with jaw surgery. I have also booked a second opinion with another experienced orthodontist. Findings: I have a significant overjet and a deep bite, with mild crowding in both arches and mild wear on the front teeth. My upper jaw is shifted to the right, and my lower jaw appears small or set back, with a retruded chin relative to the upper face. My gums and bone are generally healthy, but the lower front gum tissue is thin with some recession. Based on my own (non-clinical) measurements, my upper arch is about 30 mm between canines and 40 mm between molars, suggesting it may be narrow. I’m trying to understand whether this is a true skeletal Class II needing extraction or surgery, or whether a narrow maxilla with a functional jaw shift might be better treated with palatal expansion (MARPE/MSE).
