Q: Sofwave or Morpheus& for nonsurgical lift & tightening/more chiseled look? (Photos)

A: Thank you for your pictures and questions. I hate to be the one to tell you, but you are going to be disappointed. I own and use a Morpheus8 and I can tell you this, if you are expecting it to create "lifting and tightening" you are not going to be happy. The bottom line is that nothing creates (long-term) lifting that is not surgery and nothing creates tightening like skin removal. The treatments you are considering (M8, SW, etc.) are nothing more than invasive skin care. They might make some changes to how the skin feels, they might improve the texture and tone of the skin, but they are not going to create anything close to a "non-surgical lift." From the look of the photos, you do have a significant amount of sun damage and skin discoloration. I think that if you are looking for a non-surgical treatment that is going to create a significant improvement and something you can be happy with, I would look into Contour TRL (deep Erbium laser resurfacing). This is an aggressive laser resurfacing that takes a significant investment in terms of downtime and recovery, but you will be rewarded with a fresh new layer of skin and it will treat fine lines and wrinkles, color spots, and you might actually get some real tightening from it. Q: Explant of silicone subglandular smooth mentor implants, how much capsule to remove?

A: Thank you for this important question. In reality, the answer is up to you to choose because there is no definitive right or wrong answer. Here are some points to consider: 1) There is no good data out there to show that (outside of severe cap con or BIA-ALCL), removal of the capsule is necessary. 2) Removal of the capsule does expose the patient to increase surgical time, discomfort, and risk (especially with SM implants). 3) There is reasonably good data out there to show that self-reported patient symptom improvement after surgery was irrelevant of type of capsulectomy performed. In other words, the studies that have been done did not show that people who had a total capsulectomy did any better than those who had no capsulectomy or partial capsulectomy in terms of the symptoms they were experiencing before or after surgery. Q: Could I potentially have Symmastia? (Photo)

A: Thank you for your question and pictures. I do not see classic symmastia where the implants are connected in the middle. Things are close, but not connected. I think that it is more likely that your baseline natural anatomy had a very straight and close cleavage line prior to surgery, and that has continued after surgery, but has been amplified due to the implant volume. Q: Are there any cases of BIA-ALCL with smooth breast implants?

A: Thank you for this question. Two important distinctions to make in regard to this question. 1) To our current knowledge, has there ever been a documented case of BIA-ALCL in someone who has only ever had smooth implants? No. This may change tomorrow, but as of now, this has not been seen. 2) Has anyone with smooth implants ever been diagnosed with BIA-ALCL? Yes. This has happened, but in the patient's history, they have had some type of textured implant in the past. A: This is a poor photo to be able to answer this question and honestly it cannot really be answered from a photo anyway. At 14 months, is this likely to be cap con? No. Unless your implants at one point settled into the perfect position and then recently have begun to change. This early it is more likely a result of differences in initial pocket creation or healing to where the implants never quite settled into an ideal position over time, correct? Hope this helps!
