Facelift
Q I had a facelift 4 weeks ago today. Face is still slightly swollen.
A A good rule of thumb is that about 80-90% of the swelling will resolve in the first 4-6 weeks and the additional 10-20% can take a few months more. So be patient - especially since you are early on in your recovery. One other thing that is helpful with postop swelling is Arnica which is obtainable at any health food store. Follow the directions on the bottle. Also never fail to keep your surgeon in the loop and express your concerns to him or her. Facelift
Q My earlobes don’t match after Facelift. Any suggestions? (photos)
A It is not unusual to see minor degrees of asymmetry after facelift surgery. Give it some time to continue to heal, soften and relax and if the earlobe asymmetries persist, an earlobe reduction on the larger side can be performed to even things out. This can be achieved under local anesthesia, and most plastic surgeons do not charge for simple revisions under local. Best of luck with your continued healing and I hope you are enjoying your result

Masculinizing Top Surgery
Q I am a female to male transgender person who had a botched surgery. Can you tell me what type of surgery I should of had? photo
A Sorry to see that you have had such an unfavorable outcome. Based on the volume removed from each breast, you were likely a better candidate for a double incision approach with free nipple grafts. It appears that you still have a moderate amount of excess skin inferiorly as well as some residual breast tissue that can be removed with a revision of your surgery to the double incision approach. With the standard double incision top surgery with free nipple grafts, the contour will be superior, scar placement can be optimized into the inframammary crease and nipple placement can be achieved into a more masculine position further lateral on the pectoralis muscle. We have also had patients that have opted to not have the nipple grafts placed at the time of surgery and will then seek nipple tattooing postoperatively to place the nipples into the location of their choice. This is a decision you will have to make since you lost one of your nipples - you will either have to sacrifice the remaining nipple and have them both tattooed postoperatively or replace the surviving nipple as a nipple graft and do a nipple reconstruction on the one you originally lost. By all means place yourself into the care of a board certified plastic surgeon comfortable with FTM top surgery and more specifically top surgery revisions. Masculinizing Top Surgery
Q What's the best aftercare to reduce the appearance of post-op scarring?
A Thank you for your question. The type of scar treatment we usually recommend after top surgery really depends on the type of scar that the patient usually forms after any surgical procedure. For the patient without a history of adverse scarring (keloids or hypertrophic scars) we usually recommend any good silicone based scar gel. It is used morning and night and is used in conjunction with scar massage to keep the scars flat. With continued use the scar pinkness gradually fades and the scars become lighter. For patients that have a history of keloids we generally recommend silastic sheeting which is used as soon as the scars are healed. The sheeting is used and worn continuously except to shower (or as recommended by the manufacturer). We have also had success with IPL laser (intense pulse light laser) to get the redness out of the scars and this can be done when your surgeon determines it is safe to pursue. Q I Am a Ftm Transsexual Interested in Top Surgery but I Do Not Want my Nipples Changed?
A Thank you for your question. In my experience, most trans men (like yourself) want as tight and flat a chest as possible with as much definition to the pectoralis muscle as possible. This would not be able to be achieved with a breast reduction alone as there will still be a "pedicle" of breast tissue beneath the skin flaps that is necessary to keep the nipple areola complex alive and sensate. The reason to do the breast reduction method then becomes solely to maintain as much nipple sensitivity as possible for those trans men that postop maintenance of erotic sensation of the nipples is important. Even with this technique there is no guarantee of maintenance of nipple sensation. With the standard double incision top surgery with free nipple grafts, the contour is usually superior, scar placement can be optimized into the inframammary crease only ( as opposed to the breast reduction anchor configuration which is periareolar, vertical AND inframammary), and nipple placement can be achieved into a more masculine position further lateral on the pectoralis muscle.
