Q: Borderline lift? Getting away with just implants? (Photo)

A: You appear to have Grade II ptosis which is our fancy way of saying borderline drooping.  With moderately large implants and a dual-plane technique you should get a good result using high profile silicone or extra-high profile silicone gel implants.  The incisions could either be inframammary or periareolar.  That's the good news, and for someone who wants to avoid a lift and "doesn't mind" placement being a little low, it usually works out fine.  

Contrary to what many of the other doctors have said, I do not think you will be able to completely correct your flat upper pole, since the implants will sit relatively low.  This is not necessarily bad if you are aware and okay with that appearance.  On the other hand if you truly want excellent correction of the upper pole and implants that don't sit relatively low, I would consider a breast lift.  You should make certain to observe the difference on Vectra 3D imaging which can show the changes with specific implant styles and sizes on a three dimensional image of your chest.  It's a great way to help make this difficult decision.

Q: 4 weeks post op, how long does it takes for breast to start feeling soft?

A: Sounds like you're right on track. I tell most patients give it a solid 8-12 weeks for the breasts to demonstrate they same softness as they had before breast surgery of any type. Abnormal nipple sensation is pretty typical too. They could be extra sensitive or less sensitive - both typically resolve in a few months. 

Q: Are 500cc silicone high profile implants too big for me?

A: At 5'7" you've got a great height to handle implants in the 400+ range. Try not to get so hung up on the size. When you're in the hands of a well qualified board-certified plastic surgeon, they should be steering you in the direction of a safe choice of implant for your desired goals. Each patient consult should be a customized process. Your tissue quality, chest width, ribs, breast width, tissue thickness, nipple position...and the list goes on...all should play into selecting an optimal implant for you. Sometimes, an additional opinion can help, and many of us offer virtual consultations.

Q: When can I stop dressing my incisions?

A: It's safe to say you can stop with the dressings at this point. A silicone based scar gel would be an excellent idea, applied twice daily, for the next 6 months. All my best in your recovery. Sound like you're on track.

A: You have both. You have a double bubble = bottom of your implants are lower than your old fold...the fold used to be where your muscle was...and now the old fold is accentuated when you flex your muscle. Based on your photos, you look like you might have had a tuberous breast deformity, which I think is best corrected by placement of implants ABOVE the muscle to really stretch the tight tuberous breast out. Switching your implants to a new location above the muscle would fix this problem. You might also wish consider controlling the slight larger, puffy appearance of your areola with a small periareolar mastopexy. Things to think about. Please always make sure you are in the hands of a plastic surgeon who is board-certified by the American Board of Plastic Surgery. All the best.
