Dr. James M. Ridgway is a double board-certified plastic surgeon in Bellevue, Washington, where he provides premiere cosmetic, restorative and reconstructive surgery of the face for patients from across the greater Seattle area. He completed a fellowship at the prestigious Beckman Laser Institute, receiving various honors and accolades from The Triological Society, the American Broncho-Esophagological Association, the American Head and Neck Society, and the American Society for Lasers in Medicine and Surgery. This was followed by his acceptance into the highly competitive fellowship Facial Plastic and Reconstructive Surgery program at the University of Washington, where he honed his skills. Dr. Ridgway is board-certified by both the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology - Head and Neck Surgery. He is a member and fellow of the American Academy of Facial Plastic and Reconstructive Surgery, the American Academy of Otolaryngology-Head and Neck Surgery, the American College of Surgeons, and the European Academy of Facial Plastic Surgery. A highly respected authority in his field, Dr. Ridgway is honored to have been featured in numerous medical journals and textbooks. He is one of the founding members of the Swedish Medical Center’s Facial Reconstruction Team and is involved in various organizations and committees including Seattle Anesthesia Outreach, Global Surgical Outreach, and FACE to FACE, through which he provides voluntary surgical care for domestic violence survivors.

Q: I have a small chin. Could I increase the vertical height of my chin using fat transfer?

A: Dear Cherry Jagar, Thank you for your question. Vertical length can be created using fat transfer, but the dimension of length becomes more challenging the greater the vertical increase due to graft survival. Silicone (not liquid, but solid implants) can create a more uniform shape that has definition, feels like your skeleton and is non-mobile. Hope this help you. Be healthy and be well, James M. Q: Can a lip reduction to the center of the lip improve upper teeth show?

A: Dear an132, Thank you for your question. Two other items that are helpful to know and see is how your lips rest (slightly open) and to feel if there is a large outcropping of bone right at the center of your nose. This would be at the base of the columella (this this the tissues right between the two nostrils). The reason for this is to see if your upper lip is restricted in motion by bone versus lip excess. A: Dear heartbrie1998,

Thank you for your question in the images you have provided.  The images that you have provided classically show "ptosis" of a chin fat pad.  What this means is that when you smile the fat pad rotates down and back, off of the "bony" chin of the jaw. The correction of this is a surgical procedure with meticulous reduction of the fat pad as well as reduction of skin along the chin. The scar is hidden under the chin and out of sight. If this is something that you wish to address, then you need to see somebody who specializes in the treatment of the face as well as the review their specific before and after images for this specific type of procedure.  I hope this helps you find the best answer for you.

A: Dear Sanawm,

Thank you for your question. From the photo you provided you have a nasal hump that would need to be reduced to give a nice and natural aesthetic line. Placement of a silicone 1.5 mm implant will only make the whole nasal bride higher. Infections are not related to thickness, they are related to biologic contamination and resulting biofilm formation (1.5 mm implants can become infected as well, but there is a lower chance of extrusion/breaking through the skin). If cartilage is prepped and placed in appropriate fashion, there will be no bumpiness. Further, if the cartilage is prepped appropriately and wrapped in fascia, then there is no reason to have fat from the stomach placed.  Further, stomach fat, compared to other sites, can grow in dimension if you gain weight, hence, harvesting from a less metabolically labile site is better than from the stomach. Finally, an closed or open approach are viable and both can create an excellent result. I hope this helps you find the right answer for you.

A: Dear Gorgeousified,

Thank you for your question. The question you ask is a very reasonable one and the short answer is that you should seek medical advice as soon as you can, preferably from the provider who performed your surgery. Extrusion of an implant can happen, but the AlloDerm that you had placed should be able to integrate into normal tissue and prevent such an event.  If there is other foreign material in the nasal tip, then extrusion can happen at any point in your life - often occurring after nasal trauma.
