Dr. Kian Karimi, MD, FACS is a facial plastic surgeon in Los Angeles, California. He is double board-certified by the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology - Head & Neck Surgery. Nobody suspects a thing. I just look 10 years younger

I am completely pleased with the work Dr. Karimi did on my face and eyelids. I am a man in my early 60s. My greatest concern was that I would look like I had been “worked on” and that people would suspect that I had plastic surgery. I am pleased to report that nobody suspects a thing. A: Based on the photos you provided, you would be a great candidate for buccal fat reduction. This procedure would reduce the fat by 50-70% in the buccal region which would help you achieve more of a heart shaped face. A: You seem to be a great candidate for an upper blepharoplasty. To clarify, an upper blepharoplasty is the removal of excess skin from your upper eyelids. Since it is only the excess skin that is being removed, it should not alter the movement of your upper eyelids. Q: I have wrinkles above my upper lip. I've tried Botox but it isn't working. Is there another solution?

A: Fine lines in the perioral area are a very common concern for a lot of patients. Some of these lines are indentations in the skin which can be addressed with fillers. I’d recommend switching up from botox to dysport with a combination of fine fillers to soften up those lines. Q: Can I still get a custom chin implant if I've had a fat transfer?

A: Thank you for your question. It is possible to have a chin implant placed after the fat transfer results have settled. The chin implant will provide more projection as well as definition to your chin and jaw. Not all of the fat from the fat transfer will survive, and the final result will take some time to appear, whereas the chin implant will provide a more precise and direct result. A: A combination of procedures could be considered to address the hollowing of the lid cheek junction as well as the deep nasolabial folds. Sculptra is a great product to use in hollowing areas of the face, such as the deep nasolabial folds. This injectable is composed of poly-L-lactic acid that stimulates deep collagen production which will allow volume to be restored to the areas that it is dispersed. This injectable is typically done in sessions as it takes 6-8 weeks to see collagen restored to the face. Alternatively, hyaluronic acid fillers may also be used in the areas of concern as this provides more instant gratification. Going forward, I would recommend you to complete multiple in office consultations with board certified injectors to discuss your aesthetic goals.
