Q: Well, lower face and neck lift help my neck? (Photos)

A: You are an excellent candidate for a deep plane facelift. There is significant laxity in the neck and jowl area that could be well addressed. The procedure would require an incision just beneath the chin to deal with the deep fat and possible other tissues beneath the platysma muscle. We would aim for a mostly vertical vector in the cheeks. Repositioning the lax neck tissue requires that the incisions be placed in front of the ear (in our case behind the tragus) to create a smooth transition from the neck to the face. Medical recovery would be about 1 week with another week or two for any bruising to resolve.

A: With minor surgery you could probably obtain some good improvement. The least involved would be a submentoplasty which could be done under local anesthesia and an incision in a neck crease beneath the chin. The skin could be smoothed and some excess skin removed. If the platysma muscles were lax they could be tightened also. A more definitive but more involved procedure would be neck lift performed with incisions around the ears to remove skin from the sides. Finally there are nonsurgical options which could help. Sofwave is an ultrasound based therapy to tighten the skin and smooth it. Sylfirm X is a bipolar radiofrequency technique to both tighten and smooth the skin. Sofwave is more or less "one and done". Sylfirm X requires a series of treatments. Both are done under topical anesthesia with minimal downtime.

A: It is difficult to analyze from this photo, but it is probably scarring from acne or some trauma. This could be treated with laser resurfacing. In our practice we would use the Sciton Contour or Pro fractional laser. Radiofrequency microneedling would also be beneficial. We recommend SyldfirmX. These treatments are done under topical or local anesthesia.

Q: Blepharoplasty or brow lift? Can I do well with just eyelid surgery? (Photo)

A: This is one of the most common questions in my practice. You could achieve a very good result with just a blepharoplasty. It would open your eyes and mostly give what you want. The brows are not essential now and you could always do them later if you wished.

A: You would need an in-person exam for a definitive recommendation. Surgery would help but likely adding fat injections would be useful. These could be performed as an isolated procedure or in conjunction a facelift procedure. Likely the fat injections would be sufficient and you could do surgery later if you wished..
